老年人血管性痴呆的经颅多普勒研究

The study of Transcranial Doppler in the elderly patients with vascular dementia

目的 :探讨老年人血管性痴呆 (VD)的经颅多普勒 (TCD)改变及其实用价值。方法 :分别对 2 5例老年 VD患者 (VD组 )和 30例正常老年人 (对照组 )进行 TCD检测 ,并分析结果。结果 :与对照组相比 ,VD组患者普遍存在 TCD异常 (93.5 % ) ,主要表现为各被检血管血流速度明显降低 ,脉动指数 (PI)值增高 ,频谱异常 ,且痴呆的严重程度与脑血流速度呈正相关 ,r=0 .75 ,P<0 .0 1。结论 :老年人血管性痴呆普遍存在脑血流动力学改变 ,脑血流速度与痴呆的严重程度呈正相关 ,TCD可有效反映患者的脑血流动力学改变 。

Objective:To study the characteristic changes of Transcranial Doppler(TCD)and its practical value in the elderly patients with vascular dementia(VD).Methods:25 patients with VD and 30 normal controls were examined with TCD.The data was analyzed.Results:Compared with control group,the patients with VD showed significantly abnormal changes of TCD(93.5 %).Its mainly showed that the cerebral blood flow velocity was decreased,the pulsatility index was increased,and the frequency spectrum was abnormal.There was positive correlation between cerebral blood flow velocity and the severity of VD.Conclusion:There were significantly abnormal changes of TCD in the elderly patients with VD.The cerebral blood flow velocity was correlated with the severity of VD.By TCD,the changes of hemodynamics can be showed effectively.It has practical value in evaluating the severity of VD.
