2型糖尿病1级亲属血清维生素D水平与胰岛素抵抗指数及胰岛β细胞分泌指数的关系 被引量:7

摘要 目的探讨2型糖尿病1级亲属血清维生素D水平与胰岛素抵抗指数及胰岛β细胞分泌指数的关系。方法将纳入研究的2型糖尿病患者1级亲属根据空腹血糖(FPG)值分为4组,A组为FPG≤5.5 mmol·L-1者43例;B组为5.6 mmol·L-1≤FPG<6.1 mmol·L-1者36例;C组为6.1 mmol·L-1≤FPG<7.0 mmol·L-1者32例;D组为FPG≥7.0 mmol·L-1者31例。所有患者均计算体重指数(BMI)、腰臀比(WHR),检测FPG、空腹胰岛素(FINS)及血清25-(OH)D水平,并计算胰岛素抵抗指数(HOMA-IR)、胰岛β细胞分泌指数(HOMA-β)、胰岛素敏感指数(ISI)。结果 1)各组间血清25-(OH)D水平差异无统计学意义(P>0.05);2)D组的WHR、BMI、HOMA-IR、ISI均高于A组、B组和C组(P<0.01或P<0.05);D组的In(HOMA-β)低于A、B、C组(P均<0.01),而In(FIns)则高于A、B、C组(P<0.05)。A、B、C三组间上述指标差异均无统计学意义(P>0.05);3)血清25-(OH)D水平与FINS、HOMA-β呈正相关(P<0.05);与BMI、WHR、FPG、ISI、Homa-IR均无相关性(P>0.05)。结论 2型糖尿病患者1级亲属血清维生素D水平可能通过影响胰岛素分泌,参与了糖尿病的发生。 Objective To investigate the relationship between serum level of vitamin D and insulin resistance index, islet β cell secretion index in the first degree relatives of type 2 diabetes mellitus patients. Methods 427 first degree relatives with diabetes according to the fasting blood glucose(FPG) values were divided into four groups. FPG of group A was less than or equal to 5.5 mmol·L-1(43 cases). FPG of group B glucose was greater than 5.6mmol·L-1 and less than or equal to 6.1 mmol·L-1(36 cases). FPG of group C was greater than 6.1 mmol/L and less than or equal to 7.0 mmol·L-1(31 cases) and FPG of group D was greater than or equal to 7.0 mmol·L-1(31 cases). FPG, fasting insulin(FINS) and Serum 25-(OH) D were detected in four groups, and insulin resistance index(HOMA-IR), insulin secretion index;(HOMA-β) and insulin sensitivity index(ISI) were calculated. Result(1)The serum 25-(OH)D was no difference among groups. FPG and Homa-βwere were significantly different among groups(P<0.01). FIn and ISI were not significantly different In A, B and C group(P>0.05). FIn and ISI in group D were significantly different from those in group A, B and C(P<0.05).(2)The vitamin D was positivly correlated with FINS and HOMA-β(P<0.05), and was no correlation with BMI,WHR, FPG,ISI and Homa-IR(P>0.05).Conclusion Serum vitamin D levels of type 2 diabetes 1 relatives may participate in the occurrence of diabetes.by influencing insulin secretion.
