摘要
目的 观察面神经锐器伤后第3~5天的组织,以期确定面神经吻合时间范围。方法 取10只大耳白兔切断其双侧面神经,分别于第3天,第5天取其面神经断端近远中侧冰冻切片,应用Ache组织化学方法温育染色,照相。应用体视学方法对断端双侧切片定量分析。计算神经纤维溃变密度及阳性纤维面数密度。结果(1)面神经断后3d,近侧端神经溃变,但数量较少,溃变密度0.182±0.05;阳性纤维面数密度(0.119±0.004)根/μm^2。远侧端溃变神经数目较近侧端多,溃变密度为0.197±0.04,阳性纤维面数密度(0.123±0.004)根/μm^2。(2)面神经断后5d,近侧端神经溃变密度为0.186±0.04,阳性纤维面数密度(0.124±0.003)根/μm^2,远侧端神经溃变数量增加溃变密度为0.62±0.003,阳性纤维面数密度(0.128±0.009)根/μm^2。结论 (1)面神经断后第3天,神经近远侧端溃变无明显区别,可以进行神经吻合。面神经断后第5天,远侧端神经溃变数目较近侧端多,但可以通过神经染色选择性吻合神经断端。(2)锐器伤后5d以内可以进行神经端端吻合。
Objective Observe incision of facial nerve for 3d and 5d in order to find out a scope of time in nerve anastomosis. Methods This experiment was divided into three groups: Group Ⅰ , the separated Japan rabbits facial nerve immediately, Group Ⅱ , the Japan rabbits that their facial nerve had been separated for three days. Group Ⅲ , the Japan rabbits that their facial nerve had been separated for five days. The proximal end and distal end of nerve were respectively stained by Achehistochemical. method and analysed in quantitative. Results Separated nerve fibers for 3 days, there is little degeneration in proximal end DD: 0. 182±0.05, PFD: 0. 119±0.004 roots/μm2 Contrast with distal end DD: 0. 197±0. 04, PFD: 0. 123±0. 004 roots/μm2. separated nerve fibers for 5 days, its proximal end DD: 0. 186±0.04, PFD: 0. 124±0.003 roots/μm2 while distal end DD: 0.62±0.003, PFD: 0. 128±0.009 roots/μm2. Conclusion (1) Separated nerve fibers for 3 days, there was no difference between proximal end and distal end which can be anastomosed, separated nerve fibers for 5 days degeneration of distal end was more than proximal end but it still can be selectively anastomosed. (2) Incision of facial nerve can be anastomosed within 5 days.
