基于数据挖掘的王新志治疗脑梗死恢复期用药规律研究

Study on Medication Rules of WANG Xinzhi for Treatment of Cerebral Infarction Convalescence Based on Data Mining

目的分析王新志教授治疗脑梗死恢复期的辨治经验及用药规律,为临床治疗提供参考。方法收集王新志教授2012年1月-2017年11月于河南中医药大学第一附属医院脑病科门诊治疗的西医诊断为脑梗死恢复期/后遗症期、中医诊断为中风的医案。采用中医传承辅助平台(V2.5)分析高频药物、常用药对、核心组合、新处方等。结果收集处方883首,涉及药物250味,其中高频(>100次)药物35味,有甘草、川芎、石菖蒲等;挖掘得到2～3味药物组合35组、关联规则43条,演化得到核心组合36组、新处方16首。结论王新志教授治疗脑梗死恢复期多用祛风、化痰、散瘀、解郁、补虚之品。本研究得出的用药规律与王教授标本同治,补气养阴直至病本,祛痰化瘀、疏肝理气以疗标实的临证经验相符,可为临床提供参考。

Objective To investigate the experience in treatment and medication rules of Professor WANG Xinzhi for the treatment of cerebral infarction convalescence;To provide references for clinical treatment. Methods Medical cases diagnosed as a cerebral infarction convalescence/sequelae and stroke by TCM treated by Professor WANG Xinzhi from January 2012 to November 2017 in the Department of Encephalopathy of the First Hospital Affiliated to Henan University of Chinese Medicine were collected. High-frequency medicine, common medicine pairs, core combinations, and new prescriptions were analyzed by using TCM Inheritance Support Platform(V2.5). Results Totally 883 prescriptions consisted of 250 kinds of Chinese materia medica, of which 35 were used more than 100 times, including Glycyrrhizae Radix et Rhizoma, Chuanxiong Rhizoma and Acori Tatarinowii Rhizoma;The data mining obtained 35 groups of 2 to 3 medicine combinations and 43 association rules, and evolved to obtain 36 core groups and 16 new prescriptions. Conclusion Professor WANG Xinzhi’s treatment for cerebral infarction convalescence mainly applies medicine with efficacy of dispelling wind, dissipating phlegm, dissipating blood stasis, relieving depression and reinforcing deficiency. The medication rules obtained by this study accord with experience of Professor Wang of treating both the incidental and fundamental aspects, replenishing qi and nourishing yin, dispelling phlegm and removing blood stasis and soothing liver and regulating qi, which can provide references for clinic.
