CT导向^(125)I植入联合支气管动脉灌注治疗中晚期中心型肺癌的临床分析 被引量:11

摘要 目的探讨CT导向下癌组织内植入放射粒子125I联合支气管动脉灌注化疗治疗中晚期肺癌的临床价值。方法回顾分析经病理证实的中晚期中心型肺癌42例,先行CT引导下放射粒子125I植入,1周内经支气管动脉进行药物灌注化疗。比较治疗前后6-12个月肿瘤变化,判定疗效。结果 42例患者50个病灶,治疗6个月后随访,CR14.3%(6例)、PR71.4%(30例)、NC9.52%(4例)、PD 4.76%(例),总有效率(CR+PR)85.7%,总局控率95.2%。并发症:放射粒子植入术中:8例出现少量气胸,9例肺内少量渗血,6例术后1周内痰中带血丝。结论癌组织内植入放射粒子125I联合局部化疗治疗中晚期肺癌安全、微创,近期疗效明显。 Objective To evaluate the clinical effect of percutaneous radioactive particles 125 I implantation by CT-guided treatment bronchogenic arterial for advanced central-type lung cancer.Methods In this wook,there were 42 cases of lung cancer that proved by pathology examine.All the 25 advanced central-type lung cancer lesion from 21 patients were treated by CT-guided percutaneous radioactive pieces 125 I implantation.After a week chemothrapy implantation,medicament was injected through the arteries which provided blood to the cancer.We determined the healing effect through compareing the changes of tumours after 6months and 12 months.Results Among 25 lesions in 21 cases,following up for six months,6cases were CR(14.3%),30 cases were PR(71.4%),4cases were NC(9.52%),2cases were PD(4.76%),OR was85.7.%,the total clinical response rate was 95.2%.Among the opreation that 125 I granule was planted,a smallamount of pneumothorax was found in 8cases,a small amount of lung bleeding was found in 9cases.In one week after the operation,there was 6cases of phlegm with blood.Conclusion Percutaneous radioactive particles 125 I implantation by CTguided treatment combined with chemothrapy for advanced lung cancer has positive clinical effects in advanced central-type lung cancer with minimal damage.
