Monitoring and Management of Vascular Crisis of Free Skin Flap

目的:探讨游离皮瓣血管危象的发生机理、监测和处理方法。方法:对32例游离前臂皮瓣中出现的5例血管危象进行分析,介绍各种术后监测方法和危象皮瓣的探查、抢救措施。结果:5例皮瓣血管危象中4例经抢救成活,1例失败,抢救成功率为80％,游离皮瓣移植成活率为96.9％。结论:术后严密监测皮瓣变化,发现皮瓣血管危象后及时探查是保证危象抢救成功的关键。

Objective:To discuss the pathogensis, monitoring and management of vascular crisis of free skin flap. Methods: 5 cases vascular crisis of free skin flap were retrospectively analyzed. The multiple methods for monitoring were introduced. Surgical exploration for failing flap was described in detail. Results:4 cases vascular crisis of free skin flap were successfully salvaged, with a success rate of 80%. Conclusions:Keeping close watch over the change of flap after operation and exploring in time after vascular crisis are still the main factors that ensure the success of salvation.
