开放手术和腔内治疗前列腺增生症的经济学分析

Economics Analysis of Celiotomy and Intracavitary Operation for BPH Patients

目的 探讨前列腺增生症 (BPH)开放手术和腔内治疗费用差异及意义。方法 耻骨上经膀胱前列腺切除1 2 8例 (开放组 ) ,经尿道前列腺电切 (TURP)或 /和经尿道前列腺气切 (TUVP) 1 1 9例 (腔内组 ) ,两组间住院日、拔管日、疗效、费用及分布特征与手术方式关系进行成本 -效益分析。结果 两组间疗效无显著差异 (P >0 .0 5 ) ,腔内组手术费用明显升高 ,但住院日、拔管日、住院费用均明显少于开放组 (P <0 .0 1 )。结论 医疗资源的成本

Objective To study the cost differences in two groups of BPH patients undergone suprapubic transvesical prostatectomy or TURP/TRVP.Methods 128 cases of BPH were treated with suprapubic transvesical prostatectomy and 119 cases of BPH were treated with TURP/TRVP,A comparison of the cost-effectiveness was undertaken,including days of cathete indwelling and hospitaliztion, follow-up result,cost financial expense as related to different operative procedures.Results No significant difference of effect showed between the TURP/TUVP group and the open operative group(P>0.05). In the first group,although the operative charge was higher whereas the days of catheter indwelling and hospitaliztion were significant shortened and total expenditure was less(P<0.01).Conclusion By means of cost-effectiveness analysis of the two groups, it is shown that endourologyic technique is implying that new healthcare model is potential in minimally invasive surgery.
