摘要
目的 探讨糖尿病神经根病变的存在及H反射对糖尿病神经根病的诊断作用。方法 应用经典的H反射和周围神经传导检测方法对正常对照组30例及糖尿病组42例进行对比研究。结果 糖尿病组神经传导速度较正常组减慢,异常率为66.7%,感觉神经传导速度异常率高于运动神经传导速度。H反射的异常率为85.7%。结论 H反射较常规周围神经传导速度检测具有更高的诊断敏感性,与远端神经病变相比较糖尿病近段神经病具有相当高的患病频率。
Objective To determine the clinical application of H-reflex in the diagnosis of diabetic radiculopathy. Methods Soleus H-reflex and neural conduction velocity (NCV) were performed in 30 normal subjects and 42 diabetic patients. Results NCV was decreased with an abnormality rate of 66.7%. The sensory conduction velocity (SCV) was more sensitive than motor conduction velocity(MCV). The abnormality rate of H-reflex was 85.7%. Conclusions Among H-reflex, sensory and motor nerve conduction studies, H latency(HL) was approved to be the most sensitive parameter in diagnosing diabetic neuropathy. A high incidence rate of diabetic radiculopathy was confirmed to be comparable to that of proximal neuropathy and distal symmetrical sensory polyneuropathy.
