肺心病与COPD患者计量诊断的研究

摘要 本文通过118例经右心导管检测的肺心病及COPD患者的临床表现、胸部X线及心电图各项指标的统计分析。选择有显著意义的12项指标作为鉴别诊断因素,利用尤度法确定各因素的权重,借助于Bayes公式确定隶属函数,通过综合评判法及最大隶属原则制成肺心病与COPD鉴别诊断表,对疾病进行鉴别诊断。经临床实践诊断率92.5%显著高于医生的诊断率75.6%。所用诊断指标用常规无创法便于取得,对基层单位作为鉴别诊断的辅助方法,有一定实用价值。 Cardiac microtube examination wns performed in 118 patients with COPD and cor pulmonale. We take 12 target twelve items from those of the clinic picture, chest film and ECG, of remarkable statistical significance were chosen as differential diagnosis factors. The method of maximum likelihood method was used to affirm the weight of each factor. By means of Bayess formula the membership function was established was also established.A measure table of diagnosis according to the method of multifactorial evaluation and principle of the maximum membership. In clinic practice, the results of diagnosis made by the measure table were betterthan those by the doctor (accuracy rate of diagnosis: 92.5% versus 75.6%). Thefactors used diagnosis were easily obtainable with conventional uninvasive examina-tion. There is not equipment of cardiac microtube in small places, countrysides andmountainous areas. This method of diagnosis is practically helpful in the diagnosisof early pulmonary heart disease, especially in areas with limited equipments.
