肺曲菌球20例分析

Analysis of 20 Cases of Pulmonary Aspergilloma

本文报告肺曲菌球20例,全部经病理证实。平均年龄38岁,男:女为4:1。本病以咯血为主要临床症状。并对其X线表现进行了分析,典型X线征象是空洞内球块围以半环形气带。不典型病变随肺部原有病变不同而异。本病容易误诊和漏诊,临床上注意与结核及癌性空洞相鉴别。治疗以肺切除疗效满意。空洞内灌注抗真菌剂有一定疗效,适于肺功能低下者。

Twenty patients with pulmonary aspergillomas were reported. All were confirmedby pathology. The mean age was 38 years, the ratio of male to female was 4:1.Hemoptysis was the main symptom in patients with pulmonary aspergilloma. Thetypical radiological sign was intracavitary mass with a surrounding crescent of air.Atypical signs varied with the underlying pulmonary lesions. Misdiagnosis and misslook were often. Aspergilloma should be differentiated from tuberculous and cancer-ous cavities. The result of surgical resection was satisfactory for pulmonary aspergi-lloma. Intracavitary instillation with antifungal agents might be of help in patientsunfit for operation.
