应用介入性超声诊断治疗腹腔脏器囊肿:附103例报告

Diagnosis and treatinent of abdominal viscera cysts with interventional ultrasounography:a report of 103 cases

目的 探讨介入性超声在腹腔脏器囊肿诊断治疗中的临床应用价值。方法 ( 1)对 10 3例腹腔脏器囊肿患者采用超声引导经皮穿刺针吸细胞学检查 ,其中 2 8例进行手术病理对照 ;( 2 )对 86例进行超声引导经皮穿刺抽液后注入 99.5 %乙醇治疗或置管引流治疗。结果 ( 1)超声引导经皮穿刺针吸细胞学检查对腹腔脏器囊肿的诊断率 ( 95 .2 % )明显高于单纯超声扫描的诊断率 ( 85 .4% ) (P<0 .0 5 ) ;其中 2 8例进行手术病理对照 ,诊断符合率为 96.4%。 ( 2 ) 86例进行超声引导穿刺抽液注入99 .5 %乙醇治疗或结合置管引流治疗者 ,经 0 .5～ 5年的随访 ,囊肿消失 46例 ( 5 3 .5 % ) ,缩小 3 4例( 3 9.5 % ) ,有效率达 93 .0 %。结论 介入性超声技术简单、快捷、安全、有效 ,对机体影响小 ,是诊断和治疗腹腔脏器囊肿的有效方法。

Objective To investigate the value of clinical application of interventional ultrasounography in the diagnosis and treatment of abdominal viscera cysts(AVCs). Methods (1)One hundred and three cases of AVCs were examined by percutaneous fine-needle aspiration biopsy(FNAB) guided by ultrasonnography;the cytologic results were compared with the pathological results after operation in 28 cases. (2) The AVCs were treated with injection of 99.5% ethanol after aspiration or draining with a tube gnided by ultrasounography. Results (1) The diagnosis rate in FNAB group(91.5%) was significantly higher than that in single ultrasounogrophy group(85.4%) (P<0.05). The coincident diagnosis rate was 96.43% between the cytologic and pathological results in the 28 cases. (2) Eighty-six cases treated by injection of ethanol were followed up for 0.5～5 years after injection with of without drainage,cysts disappeared in 46 cases(53.5%) and decreased in 34 cases(39.5%). The success therapy rate was 93.0%. Conclusions Interventional ultrasounography is a simple,safe and effective way in the diagnosis and treatment of AVCs.
