“Y”形钢板内固定结合CPM治疗肱骨髁部粉碎性骨折

Treatment of humeral condylar comminuted fractures using Y-shaped plate fixation combined with CPM exercise

目的 探讨Y形钢板内固定结合CPM功能锻炼治疗肱骨髁部粉碎性骨折的疗效。方法 19例骨折按AO/ASIF分类法 :A3 型 7例 ,C1型 7例 ,C2 型 4例 ,C3 型 1例。均采用肘后正中纵形切口 ,骨折复位后用Y形钢板内固定 ,术后结合上肢CPM进行功能锻炼。结果 平均随访 13个月 ,按Aitken和Rorabeek标准进行肘关节功能评定 ,优 14例 ,良 4例 ,可 1例 ,无差病例。总优良率为94 7%。无骨不连、骨折畸形愈合、骨化性肌炎、迟发性尺神经炎、内固定失败等发生。结论 采用Y形钢板内固定结合CPM功能锻炼治疗肱骨髁部粉碎性骨折具有固定牢靠、操作简单。

Objective To evaluate the outcome of humeral condylar comminuted fractures treated by Y shaped plate fixation combined with CPM exercise. Methods 19 cases with humeral condylar comminuted fractures were classified according to AO/ASIF, A 3 in 7 cases,C 1 in 7 cases,C 2 in 4 cases,C 3 in 1 cases. All of the cases underwent open reduction and Y shaped plate internal fixation followed by CPM exercises. Results All patients were followed up for mean 13 months. According to Aitken and Rorabeek elbow function rating system, the function recovery was excellent in 14, good in 4 and fair in 1. The rate of excellent and good was 94 7%. No complcations such as nonunion, malunion,ulnal neuritis,heterotopic ossfication was found. Conclusion It has advantages such as reliable fixation, easy manipulation, excellent elbow function recovery in the treatment of humeral condylar comminuted fractures with Y shaped plate fixation combined with CPM exercise.
