F波对糖尿病周围神经病诊断的意义 被引量:5

摘要 目的 :研究糖尿病患者及健康人F波 ,探讨其对诊断糖尿病周围神经病 (DPN)的意义。方法 :检测 6 8例糖尿病患者和 4 1例健康人上肢正中神经和下肢胫神经F波 ,检测参数包括F波最短潜伏期 (Flmi)、潜伏期离散度 (Fchd)、F波传导速度 (Fwcv)、F比值 (Fratio)。结果 :病例组中糖尿病周围神经病变 5 3例 ,其Flmi、Fchd、Fwcv及Fratio与健康组比较均差异有显著性 ,F波各参数异常率普遍较高 (32 .1%~ 6 6 .0 % ) ,总阳性率为 79.2 % ;糖尿病无周围神经病变 15例 ,其中Fchd增加 4例、Flmi延长 2例、Fwcv减慢 2例。结论 :F波是诊断糖尿病周围神经病的敏感指标 ,并可发现亚临床病变 ,多个参数相结合有助于提高阳性检出率。 Objective:To evaluate the diagnostic value of F wave in diabetic peripheral neuropathy (DPN) by comparing the difference in F wave between diabetes mellitus and healthy subjects. Methods:F waves of the median and tibial nerves were measured in 68 patients with diabetes mellitus and 41 healthy persons. F latency minimum (Flmi), F chronodispersion (Fchd), F wave conduction velocity (Fwcv) and F ratio were analyzed. Results:In the 53 patients with DPN, Flmi, Fchd, Fwcv and F ratio were significantly different from those in the control group. The abnormality rates of all F wave parameters were high (32.1 % to 66.0 %) and the whole abnormality rate was 79.2 %. In the 15 patients with diabetes mellitus but without peripheral neuropathy, there were 4 cases with increased Fchd, 2 cases with prolonged Flmi and 2 cases with decreased Fwcv. Conclusion: F wave parameters are sensitive indexes to the diagnosis of DPN, and may be used to detect subclinical DPN. Combined use of all F wave parameters can improve the diagnostic sensitivity.
