多普勒超声诊断早期脑动脉粥样硬化的实验研究

摘要 目的 :探讨早期脑动脉粥样硬化血流动力学参数变化规律。方法 :将 2 2只大耳白兔随机分为 2组 :15只为实验组 ,7只为对照组。应用超声多普勒于 0、4、8、12周测量兔血流动力学参数 ,并同期行病理检查。实验组内采取自身对照 ,同时与对照组采取组间对照。结果 :自身对照收缩期峰值流速 (PSV)、舒张期最小血流速度 (EDV)、时间平均流速 (TAMAX)在 4、8周均增高 (P <0 .0 5 ) ,12周仅PSV、TAMAX增高 (P <0 .0 5 )。搏动指数 (PI)、阻力指数 (RI)、收缩期最大流速与舒张期最小流速之比 (S/D) 4周时均降低 (P <0 .0 5 )。组间对照 4周时实验组PSV、EDV、TAMAX高于对照组 (P <0 .0 5 ) ;8周时仅PSV高于对照组 (P <0 .0 5 ) ;12周时EDV、TAMAX低于对照组 (P <0 .0 5 ) ,PI、RI高于对照组 (P <0 .0 5 )。实验组 8周时内膜有少量泡沫细胞沉积 ,12周时可见脂纹。结论 :血流动力学参数改变早于形态改变 。 Objective To observe the changes of cerebrovascular hemodynamic parameters in early cerebrovascular ather osclerosis using Doppler ultrasonography.Methods Twenty-two male white rabbits were randomly divided into two groups,fifteen were atherosclerosis(AS) group,seven were control(C) group.At 0,4th,8th,12 th week,hemodynamic parameters were measured in the common carotid artery using Doppler ultrasound with spectral analysis.Results In AS group,compared with those at 0 week,PSV,EDV,TAMAX were obviously increased and PI,RI,S/D were decreased at 4th week(P<0.05);PSV,EDV,TAMAX were obviously increased(P<0.05)at 8th week;only PSV,TAMAX were increased(P<0.05)at 12th week.At 4th weeks,PSV,EDV,TAMAX of AS group were higher than those of C group(P<0.05);at 8th week,only PSV of AS group was higher than that of C group(P<0.05);at 12th week,EDV,TAMAX of AS group were lower than those of C group(P<0.05),PI,RI of AS group were higher than those of C group(P<0.05).Conclusion Doppler ultrasonography is valuable in diagnosis of early cerebrovascular atherosclerosis
