高原地区非扩髓带锁髓内钉治疗开放性股骨干骨折

Therapy of Open Femoral Shaft Fracture with Unreamed Locking Intra-medullary Nail in Highland

目的 :探讨高原地区非扩髓带锁髓内钉治疗开放性股骨干骨折的临床疗效。方法 :手术自骨折近端髓腔非扩髓逆行打入导针 ,在犁状窝处钻孔导针引导下顺利植入髓内钉 ,所有病例都首先采用静力带锁。结果 :3 5例患者平均随访 14 .8个月 ,骨折全部骨性愈合 ,关节功能恢复良好。结论 :西藏高原地区由于特殊的地理环境 ,细菌生长繁殖速度慢 ,伤口感染发生率低。带锁髓内钉在高原地区的临床应用 ,是治疗四肢长管状骨骨折较理想的内固定方法。

Objective:To investigate the clinical therapeutic effects of open femoral shaft fracture treated with unreamed locking intra-medullary nail in highland.Method:In operation after the guide wire was retrogressively beat into the marrow cavity from proximal shaft and penetrate the cortical bone at piriform fossa,the intra-medullary nail was inserted into the marrow cavity under the guidance of the guide wire.The intra-medullary nail were staticly locked at first in all cases.Results:The average follow-up time were 14.8 months for 35 cases and all the fracture healed in bony pattern with all the joints being in well-function.Conclusions:Because of the unusual geographic enviroment the bacteria in highland grew and bred at a very slow speed,which resulted in a lower incidence of wound infection.The application of locking intra-medullary nail in highland is an ideal internal fixation method for the therapy of long bone fracture.
