自体游离结肠平滑肌片做内括约肌原位肛门重建术治疗低位直肠癌

RECONSTRUCTION OF ANUS IN SITU WITH FREE COLONIC SMOOTH MUSCLE SLIDE TO REPLACE THE ARTIFICIAL INTERNAL SPHINCTER IN THE TREATMENT OF LOW RECTAL CANCER

从1994年5月至1996年2月,对32例低位直肠癌患者Miles术后采用结肠平滑肌片做内括约肌,辅以人工直肠瓣和肛管直肠角的原位肛门重建术,效果满意,该术式既能达到根治目的,又能提高患者的生存质量,并对排便功能进行评价,优12例,良14例,较好6例。作者重点介绍了手术方法,并对手术适应症,移植肌片的成活,术中应注意的问题进行了讨论。

From May 1994 to Feb 1996, 32 Cases with low rectal cancer were subjected to reconstraction of anal in situ after Miles' operation, and meanwhile, free colonic smooth muscle slide used as the artificial internal sphincter muscle, and then the artificial rectal valve and anorectal angle are attached to them. the results were satisfactory. The new operation can not only lead to a radical cure, but raise the survival quality of the patients. In the postoperative evaluation of the artificial anal function in 32 cases subjected to the operation, excellent was in 12 cases, better in 14 cases, and good in 6 cases. The surgical method and the indication were introduced in the paper. the viability of transplantuted smooth muscle slide, the problems need attention in the operation were discussed.
