无创通气治疗重症支气管哮喘21例临床分析

The clinical analysis of 21 cases of severe asthma treated by noninvasive ventilation

目的 探讨无创通气在重症支气管哮喘治疗中的应用价值。方法 应用BiPAPViSION型呼吸机经鼻 /面罩双水平气道正压通气 (BiPAP)治疗 2 1例重症支气管哮喘患者 ,分析其通气治疗前后临床表现及动脉血气的变化 ,并以同期同等病情程度 2 1例患者作对照。结果 BiPAP治疗 4h后所有患者呼吸困难明显缓解 ,呼吸频率由 (31± 5 )次 /min降至 (2 5± 4 )次 /min(P <0 .0 5 ) ,心率由 (1 32± 9)次 /min降至 (1 1 1± 1 3)次 /min(P <0 .0 5 ) ,动脉血氧分压 (PaO2 )、CO2 分压 (PaCO2 )均明显改善 (P <0 .0 5 ) ,治疗总有效率为 1 0 0 %。无 1例发生严重并发症。结论 应用无创通气治疗重症支气管哮喘效果好 ,并发症少 ,患者易于接受 ,值得临床推广。

Objective To investigate the value of noninvasive ventilation in treating patients with severe asthma.Methods Twenty one cases of severe asthma were treated with BiPAP ViSION via facial/nazal mask.The clinical presentation and arterial blood gases were analysed before and after ventilation.Twenty one cases of the same severe asthma were served as controll during the same period.Results All of these patients improved in dyspnea,respiratory rate (RR),heart rate(HR),PaO 2 and PaCO 2 4 hours after ventilation and RR decreased from (31±5)bpm to(25±4)bpm( P <0.05),HR decreased from(132±9)bpm to (111±13)bpm( P <0.05).The effective rate was 100%.No one developed severe complication.Conclusion Noninvasive ventilation in treating severe asthma is effective,well compliant and has few complication.
