摘要
目的 探讨卵泡发育及生殖道局部白细胞介素 1β (IL 1β)与黄体功能不全患者不孕的关系。 方法 对 2 1例诊断为黄体功能不全的患者 ,进行B超监测卵泡发育 ,放射免疫法 (RIA)检测排卵期血清、宫颈粘液中IL 1β的表达 ,用化学发光免疫分析仪测定黄体期血清孕酮水平 ,并与 18例正常组作对照。结果 黄体功能不全患者 (LPD组 )与对照组优势卵泡直径比较差异有显著性 (P <0 0 5) ,两组黄体中期血清孕酮比较差异有显著性 (P <0 0 1) ,优势卵泡直径与黄体中期血清孕酮呈正相关 (P <0 0 1)。排卵期LPD组血清IL 1β与对照组比较无显著性差异 (P >0 0 5) ,而宫颈粘液中IL 1β与对照组比较差异有显著性 (P <0 0 5) ,且宫颈粘液中IL 1β与黄体中期血清孕酮呈负相关 (P <0 0 1)。结论 排卵期生殖道局部IL 1β的高表达可能是黄体功能不全患者不孕的发病环节中的主要免疫因素之一 。
Objective To study the relationship between infertility due to luteal phase defect and o varian follicular development and the levels of interleukin-1β (IL-1β) in se rum and cervical mucus. Methods Twenty-one infertile patients with luteal phase defect (LPD group) and 18 normal fertile women (control group) were subjected to B-ul trasonography for ovarian follicular development. Serum and cervical mucus level s of IL-1β were determined by radioimmunoassay (RIA). Serum progesterone (P) l evels from the midluteal phase were measured by access immunoassay system. Results The maximum follicular diameter was significantly small er in the LPD group than in the control group (P<0 05). Serum P level from the midluteal phase in LPD group was significantly lower than in control group ( P<0 01). There was significant positive correlation between maximum follicu lar diameter and midluteal serum P (r=0 691, P<0 01). IL-1β levels in ce rvical mucus were significantly higher than in serum (P<0 01). During ovula tory phase the levels of IL-1β were similar in serum of control group and LPD group (P>0 05). IL-1β levels in cervical mucus in the LPD group were sign ificantly higher than in the control group (P<0 05). Of great interest was the negative correlation between IL-1β levels in cervical mucus and midluteal serum P (r=-0 890, P<0 01). Conclusion The increase of IL-1β levels in genital locality m ay play an important role in infertility with LPD during ovulatory phase, and th us may affect indirectly corpus luteum resulting in LPD.
