胸主动脉瘤的外科治疗

Surgical Treatment of Thoracic Aortic Aneurysm

目的 总结 10例胸主动脉瘤的外科治疗经验。方法 在体外循环下行主动脉瓣及人造血管置换 1例 ,Ben tail手术 2例 ,Cabrol手术 1例 ,在左心转流下行降主动脉瘤切除 +人造血管移植 4例 ;中度低温停循环主动脉 /股动脉交替灌注降主动脉瘤切除 2例。结果 全组 10例无手术死亡及术后并发症的发生。结论 手术技术。

Objective To summarize the experience in surgical treatment of 10 patients with thoracic a ortic aneurysm. Methods One patient underwent resection of asc ending aneurysm with graft and aortic valve replacement under extracorporeal cir culation. Bentail procedure was performed in 2 cases and Cabrol procedure in 1 c ase. Four patients underwent excision with graft replacement of descending aorti c aneurysm by left atrium to distal descending aortic bypass and 2 resection of descending aneurysm with graft by aortic and femoral artery perfusion alternativ ely under moderate hypothermia with circulatory arrest. Results There was no operative dead and postoperative complicat ion in all of the 10 cases. Conclusion Surgical procedure, selection of the extracorporeal circulation methods and the prevention of main organs from ischemia-induced dam age are the key for successful operation.
