腓肠神经营养血管蒂岛状皮瓣的临床应用

Clinical application of sural neurovascular pedicle island flap

目的探讨应用腓肠神经营养血管蒂岛状皮瓣修复小腿、踝部和足部皮肤软组织缺损的方法。方法2001-09～2006-10,应用腓肠神经营养血管蒂岛状皮瓣修复小腿、踝部、足部皮肤软组织缺损15例,其中创伤12例,深度烧伤2例,冻伤1例。结果15例患者采用该皮瓣修复后全部成活,随访半年,皮瓣修复后形态、血运、质地、弹性佳,踝关节功能恢复良好。无溃疡发生。结论腓肠神经营养血管蒂岛状皮瓣具有血管恒定,变异少,血运可靠,皮瓣成活率高,手术操作不复杂,不牺牲主要动脉,是一种修复下肢皮肤软组织缺损较好的手术方法之一,值得在临床上推广应用。

Objective To explore the clinical application of sural neurovascular pedicle island flap to repair soft-tissue of the leg 、 ankle and foot.Methods From sep.2001 to Oct.2006,15 patients with soft-tissue defects in the leg 、 ankle and foot were repaired by using of sural neurovascular pedicle island flap ineluding 12 cases of traumatic defects,2 cases of burns and 1 case of frostbite.Results All the flaps are survive.Postoperative follow-up for 6 months the circulation,color and texture of flaps were excellent.The appearance and function of ankle joints were good.No ulcer happened.Conclusion This flap has sufficient blood supply and a high survival rate,It is convenient in design,dissection and without sacrifice of major arteries.So it is an effective method for the reconstruction of soft-tissue defects in ankle and foot.
