老年2型糖尿病患者甲状腺激素水平变化的临床意义 被引量:3

摘要 目的探讨老年2型糖尿病患者甲状腺激素水平变化的临床意义。方法选取2017年6月至2018年6月我院收治的160例老年2型糖尿病患者为研究组,另选取同期160例健康者为对照组。通过放射免疫分析法分别检测研究组和对照组的血清游离三碘甲状腺原氨酸(FT_3)、促甲状腺激素(TSH)和游离甲状腺激素(FT_4),比较两组的FT_3、 TSH、 FT_4水平,比较不同程度2型糖尿病患者的FT_3、 TSH、 FT_4水平。结果研究组的血清FT_3水平显著低于对照组,血清TSH水平显著高于对照组,差异有统计学意义(P <0.05)。2型糖尿病患者中,重型组和中型组患者的血清FT_3水平显著低于轻型组,血清TSH水平显著高于轻型组患者,差异有统计学意义(P <0.05)。结论甲状腺激素水平变化能够反映老年2型糖尿病患者的病情,病情越严重,血清中FT_3水平越低,而TSH水平越高。在老年2型糖尿病患者中进行甲状腺功能筛查,对于帮助患者及早发现自身甲状腺激素水平异常情况并采取相应的治疗措施具有重要的临床意义。 Objective To explore the clinical significance of changes of thyroid hormone levels in elderly patients with type 2 diabetes mellitus.Methods 160 cases of elderly patients with type 2 diabetes mellitus admitted to our hospital from June 2017 to June 2018 were selected as the study group,and the 160 cases of healthy subjects at the same period were selected as the control group.Serum free triiodothyronine(FT3),thyroid stimulating hormone(TSH)and free thyroid hormone(FT4)in the study group and control group were measured by radioimmunoassay,and the levels of FT3,TSH and FT4 were compared between the two groups.Also,the levels of FT3,TSH and FT4 of patients with different degrees of type 2 diabetes mellitus were compared.Results The serum FT3 level of the study group was significantly lower than that of the control group,and the serum TSH level was significantly higher than that of the control group(P<0.05).Among patients with type 2 diabetes mellitus,the severe group and moderate group had significantly lower serum FT3 level and significantly higher serum TSH level than the mild group(P<0.05).Conclusions The changes of thyroid hormone levels can reflect the disease condition of elderly patients with type 2 diabetes mellitus.The more severe the condition,the lower the serum FT3 level,and the higher the TSH level.Screening for thyroid function in elderly patients with type 2 diabetes mellitus has important clinical significance for helping patients detect abnormal levels of hyroid hormones and take appropriate treatment measures.
