摘要
用结扎左冠状动脉前降支的方法,造成家兔急性心肌梗塞,观察结扎后1/2至150分钟心脏舒张性能的动态变化,并与心脏收缩性能及心外膜电图 ST 段的改变相比较,结果发现:1.急性心肌梗塞时心脏舒张性能指标-dp/dt max 下降、T 值延长,与结扎前相比差异非常显著(P<0.01);2.心脏舒张性能和收缩性能之间呈高度负相关,T 值与+dp/dt max、VCE 的相关系数分别为 r=-0.85和 r=-0.96,但 ST 段变化与心脏舒张性能之间无直接关联;3.T 值与 LVEDP 变化规律相符.结果表明,在急性心肌梗塞时,采用心脏舒张性能指标了解心泵功能的改变比收缩性能指标更敏感、更可靠.
Acute myocardial infarction(AMI) was produced by ligatation of left anterior des- cending coronary artery(LAD).Dynamic changes of cardior diastolic function were observed con- secutively from start to 150 min after LAD ligated.The relations among diastolic and systolic func- tion and ST segment changes were analysed. It was found that:(1)the decrease of negative maximal rate of rise(-dp/dt max) (p<0.01),and increase of time constant of isovolumic diastolic pressure decay(T) were signeficant (p<0.01) during AMI;(2) good correlations were present between T and positive maximal rate of rise of left ventricular(+dp/dt max) (r=-0.85) or maximum velocity of contractile element shotening(VCE) (r=-0.96).The ST segment change was not immediately associated with cardiac diastolic function. (3) Regularity of dynamic change of T was accorded with left ventricular end-diastolic pressure(LVEDP).Thus the index of cardiac diastolic function is more delicate and reliable in reflectiog cardiac pump function
