CD10和CD34免疫组化双染色在增生性肾小球肾炎病理诊断中的意义分析 被引量:2

摘要 目的分析研讨CD10和CD34免疫组化双染色在增生性肾小球肾炎病理诊断中的意义。方法随机从该院2016年4月-2018年2月期间收治的肾疾病患者116例进行讨论,根据患者疾病分组,对照组中共35例肾小球微小病变,研究组(81例)中包含新月体性肾小球肾炎21例、膜增生性肾小球肾炎25例、毛细血管内增生性肾小球肾炎12例、系膜增生性肾小球肾炎23例,两组患者均接受免疫组化双染色法肾活检,标记CD10和CD34,并分析检测结果。结果肾小球系膜基质和细胞CD10、CD34表现为(-),颜色为无色;肾小囊上皮细胞,壁层CD10表现为(+++),颜色为红色,脏层CD10(+++),颜色为红色;肾小球毛细血管内皮CD34(+++),颜色为棕色。肾小球系膜基质和细胞呈显著增生状,毛细血管基膜中存在系膜基质,肾小囊脏层上皮细胞增生突出,挤压血管管腔。新月体性肾小球肾炎、膜增生性肾小球肾炎、毛细血管内增生性肾小球肾炎、系膜增生性肾小球肾炎患者CD10、CD34检测结果均与病理结果相符。结论肾活检病理光镜检查包含马松染色、六胺银染色、过碘酸雪夫反应染色、苏木精-伊红等染色,联合CD10、CD34免疫组化双染色,有利于提升增生性肾小球肾炎病理类型和病理变化的诊断准确性,值得推广。 【Objective】To analyze the significance of CD10 and CD34 double immunohistochemical staining in the pathological diagnosis of proliferative glomerulonephritis.【Methods】A total of 116 patients with renal disease admitted to our hospital from April 2016 to February 2018 were randomly selected.According to the patient’s disease group,35 cases of glomerular micro lesions were included in the control group;in the study group(81 cases),there were 21 cases of crescentic glomerulonephritis,25 cases of membranous proliferative glomerulonephritis,12 cases of endocapillary proliferative glomerulonephritis,23 cases of mesangial proliferative glomerulonephritis,both groups recieved renal biopsy by immunohistochemical double staining,CD10 and CD34 were labeled,and the results were analyzed.【Results】CD10 and CD34 of mesangial matrix and cells were(-)and colorless;CD10 of renal capsule epithelial cells was(+++),red in color,and CD10 of visceral layer was(+++),red in color;CD34 of glomerular capillary endothelium was(+++),and brown in color.The mesangial matrix and cells of the glomerulus were markedly proliferated.There was mesangial matrix in the capillary basement membrane.The proliferation of epithelial cells in the visceral layer of the renal capsule was prominent and the vessel cavity was squeezed.The results of CD10 and CD34 in patients with crescent glomerulonephritis,membranous proliferative glomerulonephritis,endocapillary proliferative glomerulonephritis and mesangial proliferative glomerulonephritis were consistent with the pathological results.【Conclusion】The light microscopy of renal biopsy includes Masson,PASM,PAS,HE staining,combined with CD10 and CD34 immunohistochemical double staining is helpful to improve the diagnostic accuracy of pathological types and pathological changes of proliferative glomerulonephritis,and is worth popularizing.
