甲状腺自身抗体水平检测对原发性甲减的诊断价值分析

Value of thyroid autoantibody level detection in diagnosis of primary hypothyroidism

目的探讨甲状腺自身抗体水平检测对原发性甲减的诊断价值。方法选取2017年3月‐2018年2月该院治疗的94例原发性甲减患者作为观察组,选取同期于该院进行体检的94例健康者作为对照组,检测两组游离T3(FT3)、游离T4(FT4)、促甲状腺激素(TSH)、抗甲状腺受体抗体(TRAb)、促甲状腺球蛋白抗体(TGAb)及抗甲状腺过氧化物酶抗体(TPOAb)等甲状腺自身抗体指标,对比两组甲状腺自身抗体阳性率及甲状腺自身抗体各项指标检测结果。结果观察组TRAb、TGAb及TPOAb阳性率(79.79%、91.49%、17.02%)均较对照组(4.26%、6.38%、1.06%)高,差异有统计学意义(P?<0.05);观察组FT3、FT4、TSH、TRAb、TGAb及TPOAb检测结果为(2.94±1.15)pmol/L、(5.38±2.24) pmol/L、(71.82±32.26)mIU/L、(15.67±7.12)%、(29.14±10.57)IU/ml、(113.57±54.32)IU/ml,与对照组的(4.72±1.23) pmol/L、(10.75±2.86)pmol/L、(2.69±1.03)mIU/L、(0.17±0.06)%、(2.26±1.05)IU/ml、(4.35±1.89)IU/ml相比,差异有统计学意义(P?<0.05)。结论原发性甲减患者采取甲状腺自身抗体水平检测的诊断效果较显著,可作为原发性甲减诊断的重要指标,为临床诊治提供科学依据。

【Objective】To investigate the value of detection of thyroid autoantibody level in the diagnosis of primary hypothyroidism.【Methods】Ninety-four patients with primary hypothyroidism treated in the hospital from March 2017 to February2018 were selected as observation group,and 94 healthy people underwent physical examination in the hospital were selected as control group.The thyroid autoantibodies indexes such as free T3(FT3),free T4(FT4),thyroid stimulating hormone(TSH),antithyrotropin receptor antibody(TRAb),thyroglobulin antibody(TGAb),anti-thyroid peroxidase antibody(TPOAb)in the two groups were detected.The positive rate of thyroid autoantibody and detection results of thyroid autoantibodies indexes were compared between the two groups.【Results】The positive rate of TRAb,TGAb,TPOAb in observation group(79.79%,91.49%,17.02%)was higher than that in control group(P<0.05);the detection results of FT3,FT4,TSH,TRAb,TGAb,TPOAb in observation group[(2.94±1.15)pmol/L,(5.38±2.24)pmol/L,(71.82±32.26)mIU/L,(15.67±7.12)%,(29.14±10.57)IU/ml,(113.57±54.32)IU/ml]and control group[(4.72±1.23)pmol/L,(10.75±2.86)pmol/L,(2.69±1.03)mIU/L,(0.17±0.06)%,(2.26±1.05)IU/ml,(4.35±1.89)IU/ml]were compared,and there were statistical differences(P<0.05).【Conclusion】The detection of thyroid autoantibody level in the diagnosis of primary hypothyroidism has significant effects,and it can be taken as an important indicator for the diagnosis of primary hypothyroidism,providing a scientific basis for clinical diagnosis and treatment.
