摘要
目的:探讨动脉瘤的血流动力学及其与动脉瘤发展之间可能的相互关系。材料和方法:用静脉囊镶嵌法将18条犬制成三种类型(单侧型、分叉型、末梢型)颈总动脉瘤模型,术后2周、4周分别用彩色多普勒(ATLUltramark9型)检测载瘤动脉、动脉瘤的血流方向、状态,及其血流参数(MaxV、MinV、TAV、PI、RI值),进行方差分析和配对t检验。结果:彩色多普勒可发现动脉瘤颈部的MaxV、MinV、TAV值低于载瘤动脉主干,但RI、PI值较之增高;动脉瘤的远侧缘、底部、近侧缘呈红色的层流,瘤中央呈红、兰相镶的湍流;动脉瘤颈部、远侧缘、底部、近侧缘的MaxV、MinV、TAV值4周低于2周,RI值却增高,并二者比较,差异有非常显著性(P<0.001)。结论:彩色多普勒检测实验性动脉瘤有利于了解动脉瘤发展、血栓形成或破裂的血流动力学。
Purpose :To study the probable relationship between the hemodynamics and the development of aneurysm by Color Doppler flow imaging. Materials and Methods:The common carotid aneurysm formats were made in 18 canine by venous bulbar wedging method,dividing into 3 types(Unilateral 6,bifurcation 6,and terminal 6),2 weeks and 4 weeks postoperative follow up by Color Doppler (ATL Ultramark 9 Type) were evaluated respectively in displaying the parentartery, the hemodynamics and flow parameter of the aneurysms(Max V、 Min V、 TAV、 PI、 RI Value) and finally completed with analysis and conjugation tests. Results: Color Doppler could discovered the Max V、Min V、TAV at the neck of the aneurysm were lower than those of the parent artery but somewhat higher in Rl and PI values, the distal border, fundus and proximal border presented as red colored layerings while the central part of the aneurysm showing red and blue partition turbulents. The Max V、Min V、TAV of the neck, distal border, fundus and proximal border were lower at 4 week than those of 2 weeks postoperatively but higher in RI valus with obvious disparity (P<0.001).Conclusion: Color Doppler flow imaging of the experimental canine aneurysms gave a prospective evaluation on development, thrombus formation and rupture hemodynamics.
