2013-2014年北京市朝阳区接种含麻疹成分疫苗后发热出疹病例特征分析

The Monitoring Features in Rash and Fever Illness after Measles Vaccine Inoculation in Chaoyang District of Beijing,2013-2014

目的通过对麻疹专病监测报告系统和疑似预防接种异常反应(AEFI)监测网络直报管理系统中接种含麻疹成分疫苗(MCV)后发热出疹病例(RFIs)监测数据分析,了解MCV后RFIs的发病特征。方法对北京市朝阳区2013-2014年通过麻疹专病监测报告系统和AEFI监测系统上报的所有MCV后RFIs病例进行流行病学调查和分析。结果 2013-2014年麻疹专病监测报告系统和AEFI监测网络直报管理系统分别报告RFIs28例和38例,其中2例重合。两系统上报的64例RFIs中男女性别比1.06∶1,年龄范围8月龄~28岁。以朝阳区MCV疫苗使用量计算,RFIs的发生率平均为20.96/10万。麻疹专病监测系统报告RFIs MCV接种与出疹间隔时间平均8.4 d;AEFI监测系统报告RFIs病例平均3.14d。麻疹专病监测系统上报的28例RFIs,经流调、采样诊断为MCV相关病例16例,麻疹4例,其他出疹性疾病8例。AEFI系统上报38例RFIs,经调查诊断为偶合症1例,一般反应20例,异常反应17例。结论麻疹专病监测报告系统和AEFI系统对RFIs的监测相对独立,重合病例较少,且两系统报告病例特征有所差异,麻疹专病监测报告系统可为AEFI系统做补充。

Objective To explore the features of rash and fever illness( RFIs) after Measles- containing Vaccine( MCV) inoculation,based on the special measles surveillance system and adverse event following immunization( AEFI) surveillance system. Methods The report data of RFIs was collected and analyzed from the special measles surveillance system and AEFI surveillance system in Chaoyang district of Beijing,2013 to 2014. Results From 2013 to 2014,there were 28 cases of RFIs in the special measles surveillance system,38 cases of RFIs in AEFI surveillance system and 2 cases were reported in both systems. The male to female ratio was1. 06: 1; the age was from 8 month to 22 years old. The incidence of RFIs was 20. 96 /100 000. The average interval between inoculation and rash of RFIs was 8. 4 days in the special measles surveillance system and 3. 14 days in AEFI surveillance system. There were 16 cases of MCV,4 cases of measles and 8 cases of other illness in 28 RFIs of the special measles surveillance system. There was 1 case of vaccine coupling,20 cases of general reaction and 17 of adverse reaction in 38 RFIs of AEFI surveillance system. Conclusion The special measles surveillance system was different from AEFI surveillance system,and the special measles surveillance system could be a supplement for AEFI surveillance system.
