跨中、后颅窝轴外肿瘤及肿瘤样病变的影像学诊断 被引量:1

摘要 目的 :提高对跨中、后颅窝轴外肿瘤及肿瘤样病变的影像学认识。方法 :60例经病理证实的跨中、后颅窝轴外肿瘤及肿瘤样病变按位置分为中央型和外侧型 ,后者又分中颅窝型、后颅窝型及骑跨型 ,分析各型构成情况及影像学特征。结果 :中央型 12例 ,脊索瘤 5例 ,垂体瘤 3例 ,鼻咽癌 2例 ,颅咽管瘤、脑膜瘤各 1例。外侧型 48例 ,三叉神经肿瘤 14例 ,脑膜瘤 12例 ,表皮样囊肿 11例 ,硬膜型海绵状血管瘤 4例 ,皮样囊肿、转移瘤各 2例 ,血管外皮瘤、颈静脉球瘤、鼻咽癌各 1例。多数病变可根据影像学特点做出诊断并明确范围。结论 :按位置特征对跨中、后颅窝轴外肿瘤及肿瘤样病变分型 ,有助于定性诊断及范围描述 。 Objective:To study the imaging features of extra axial tumors and tumorlike lesions involving both middle and posterior cranial fossa and to get a better understanding of them.Methods:60 cases of pathologically confirmed extra axial tumors and tumorlike lesions involving both middle and posterior cranial fossa were analyzed.They were devided into central and lateral types,and the latter were subdivided into three types:middle cranial fossa type,posterior cranial fossa type and the over riding type.The consititution and imaging features of each type were analyzed.Results:There were 12 cases of central type,included 5 cases of chordoma,3 cases of pituitary adenoma,2 cases of nasopharyngeal carcinoma,1 case of craniopharyngioma and 1 case of meningioma.48 cases of lateral type included 14 cases of trigenminal nerve tumors,12 cases of meningioma,11 cases of epidermoid cyst,4 cases of dural cavanous hemangioma,2 cases of dermoid cyst,2 cases of metastasis and 1 case of hemangiopericytoma,paraganglioma of glomus jugulare and nasopharyngeal carcinoma for each.In most cases the nature and extent of the lesions can be made on the basis of their imaging features.Conclusion:It is helpful to categorize extra axial tumors and tumorlike lesions involving both middle and posterior cranial fossa according to their location for evaluation of the nature and extent of these lesions.It is of great value in correct diagnosis and treatment planning of these lesions.
