缩宫素联合卡孕栓预防剖宫产术后出血的疗效

Effect of oxytocin combined with carboprost methylate suppositories on postoperative hemorrhage after cesarean section

目的观察剖宫产术中使用缩宫素并联合不同时间和方法使用卡孕栓对预防剖宫产术后出血的疗效。方法选择2015年1月至2018年3月桂林市中西医结合医院七星分院妇产科行剖宫产分娩的初产妇180例为研究对象,随机分为缩宫素组(A组)、缩宫素加卡孕栓组(B组)、缩宫素加卡孕栓组(C组),每组60例。比较3组产妇术中、术后2 h及24 h出血量,术后2 h子宫收缩情况并观察不良反应发生情况。结果与A组和C组相比,B组术中、术后2 h及24 h出血量显著减少(P<0.05,或P<0.01)。术后2 h子宫收缩良好率组建比较差异有统计学意义(P<0. 05)。三组产妇不良反应发生率差异无统计学意义(P>0. 05)。结论胎儿娩出前3 min舌下含服卡孕栓联合缩宫素能更好地预防剖宫产术后出血,值得在广大基层医院推广应用。

Objective To observe the curative effect of oxytocin combined with carboprost suppositories in different ways for preventing postoperative hemorrhage after cesarean section.Methods A total of 180 primipara who underwent cesarean section delivery from January 2015 to March 2018 were randomly divided into groups A,B and C with 60 cases each.The amount of blood loss during surgery,2 h and 24 h after operation and 2 h postoperative uterine contractions were compared and adverse reactions were observed.Results Compared with groups A and C,the amount of blood loss was significantly reduced in group B during surgery,2 h and 24 h after operation(P<0.05,or P<0.01).The good rate of uterine contraction of group A was significantly lower than that in groups B and C(P<0.05).There was no statistically significant difference in the incidence of maternal adverse reactions between the three groups(P>0.05).Conclusion Sublingual carboprost methylate suppositories at 3 minutes before fetal delivery is more effective in preventing PPH in primipara undergoing cesarean delivery.
