胺碘酮联合缬沙坦治疗慢性心功能不全合并阵发性房颤临床分析

Clinical Analysis of Amiodarone Combined with Valsartan in Chronic Heart Failure Complicatied with Paroxysmal Atrial Fibrillation

目的:探讨胺碘酮联合缬沙坦治疗慢性心功能不全合并阵发性房颤的临床效果及其对心功能的影响。方法:选择笔者所在医院2016年1月-2017年5月收治的148例慢性心功能不全合并阵发性房颤患者,将其随机分为A组及B组,每组74例,A组采用胺碘酮治疗,B组采用胺碘酮联合缬沙坦治疗,疗程6个月,比较两组患者治疗后窦性心律维持率,心功能分级变化及左心室功能参数变化。结果:治疗6个月后,B组窦性心律维持率高于A组,差异有统计学意义(P<0.05)。两组患者心功能分级均较治疗前改善,且B组优于A组,差异有统计学意义(P<0.05)。两组患者治疗前左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)及左心室射血分数(LVEF)比较,差异无统计学意义(P>0.05);治疗后两组LVESD、LVEDD较治疗前降低、LVEF较治疗前升高,且B组LVESD、LVEDD低于A组、LVEF高于A组,差异均有统计学意义(P<0.05)。结论:胺碘酮联合缬沙坦治疗慢性心功能不全合并阵发性房颤能够有效控制心律,改善心功能,具有较好的临床效果。

Objective:To explore the clinical efficacy of Amiodarone combined with Valsartan in chronic heart failure complicatied with paroxysmal atrial fibrillation and its effect on cardiac function.Method:A total of 148 patients with chronic heart failure complicated with paroxysmal atrial fibrillation in our hospital from January 2016 to May 2017 were selected.They were randomly divided into group A and group B,74 cases in each group,The patients in group A were treated with Amiodarone,and the patients in group B were treated with Amiodarone combined with Valsartan for 6 months.The maintenance rate of sinus rhythm,heart function classification and the left ventricular function parameter changes after treatment were compared between group A and group B.Result:After 6 months of treatment,the maintenance rate of sinus rhythm in group B was higher than that in group A,the difference was statistically significant(P<0.05).The heart function classification of patients in the two groups were improved than that before treatment,and group B was superior to group A,the difference was statistically significant(P<0.05).Comparison of the LVESD,LVEDD and LVEF between the two groups before treatment,there were no significant differences(P>0.05).The LVESD and LVEDD were decreased and the LVEF was increased in the two groups after treatment,and the LVESD and LVEDD in group B were lower than those in group A,and the LVEF value was higher than that in group A,the differences were statistically significant(P<0.05).Conclusion:Amiodarone combined with Valsartan can effectively control the rhythm of heart,and improve the cardiac function in chronic heart failure complicatied with paroxysmal atrial fibrillation,and it has good clinical effect.
