M5a和M5b患者实验室检查结果对比分析

摘要 目的:比较M5a和M5b患者实验室检查结果异同,对M5患者实验室检查结果进行归纳总结,为该病的临床诊断及分型提供依据。方法:收集2015年2月-2017年8月韶关市第一人民医院及粤北人民医院初诊M5患者68例,其中M5a 20例,M5b 48例,对以上患者进行血常规检测、免疫表型检测及染色体核型分析。结果:M5a患者白细胞总数及单核细胞所占比例均明显高于M5b患者(P<0.05),M5a和M5b患者RBC、HGB、HCT均有不同程度降低,但两组患者之间差异无统计学意义(P>0.05);CD3、CD10、CD20、和CD41a在M5a患者和M5b患者中阳性率均较低,且两组患者之间差异无统计学意义(P>0.05);M5b患者CD7、CD14、CD19、GlyA、CD15、CD13、CD34、HLA-DR阳性率均高于M5a患者,CD19、GlyA和CD13两者间的差异无统计学意义(P>0.05),CD7、CD14、CD15、CD34和HLA-DR两者间的差异有统计学意义(P<0.05);M5a患者CD33和CD117阳性率高于M5b患者(P<0.05);M5a患者骨髓细胞染色体核型异常者所占比例明显高于M5b患者(P<0.05),以非平衡核型异常最为明显(P<0.01)。结论:M5a和M5b患者实验室检查各具特征,对M5a和M5b患者实验室检查结果进行归纳有助于M5的诊断及分型。 Objective:To compare the results of laboratory tests of M5 a and M5 b,to summarize the results of laboratory examination of M5 patients and provide basis for clinical diagnosis and classification of the disease.Method:A total of 68 first visit cases of M5 patients were collected,including 20 cases of M5 a,48 cases of M5 b,in First People’s Hospital of Shaoguan from February 2015 to August 2017.Routine blood tests,bone marrow observation,immune phenotypic detection and chromosome karyotype analysis were all analysed for above patients.Result:The morphological features of M5 a and M5 b were not significant.Total number of white blood cells and mononuclear cells in M5 a patients were significantly higher than M5 b patients(P<0.05).RBC,HGB,HCT were reduced to some extent in all patients with M5 a and M5 b,but there were no statistically significant difference between two groups of patients(P>0.05).The positive rate of CD3,CD10,CD20,and CD41 a in M5 a patients and M5 b patients were both low,and there were no statistically significant difference between the two groups(P>0.05).The positive rate of CD7,CD14,CD19,GlyA,CD15,CD13,CD34,HLA-DR in M5 b patients were higher than that in patients with M5 a.CD19,GlyA and CD13 between M5 a and M5 b were no statistically significant difference(P>0.05).CD7,CD14,CD15,CD34 and HLA-DR between M5 a and M5 b were statistically significant difference(P<0.05).The positive rate of CD33 and CD117 in M5 a patients was higher than that of M5 b patients(P<0.05).The proportion of abnormal chromosomal karyotypes in M5 a patients was significantly higher than that of M5 b patients(P<0.05),and the non-equilibrium karyotype abnormality was most obvious(P<0.01).Conclusion:M5 a and M5 b patients have the characteristics of laboratory examination,and the results of laboratory examination of M5 a and M5 b patients can be summarized to help the diagnosis and classification of M5.
