经皮椎间孔镜手术与椎板开窗术对腰椎间盘突出症疗效对比

Comparative Analysis of Curative Effect of Percutaneous Intervertebral Foramen Microscopy and Laminectomy for Lumbar Disc Herniation

目的:对比经皮椎间孔镜手术与椎板开窗术对腰椎间盘突出症的临床疗效。方法:对2017年1月-2018年12月笔者所在医院诊治的116例腰椎间盘突出症患者进行临床观察及研究,采用数字表抽取法随机分组,对照组58例采用半椎板开窗术治疗,观察组58例,采用经皮椎间孔镜术治疗,对患者的VAS评分、JOA评分、ODI评分、手术时间、住院时间、术中出血量和术后并发症发生率等指标进行观察记录。结果:手术治疗后,观察组VAS评分明显比对照组低,并且观察组住院时间及手术时间均明显短于对照组(P<0.05),术中出血量也明显少于对照组(P<0.05)。而且观察组术后并发症的发生率显著低于对照组(P<0.05)。同时,观察组治疗后的JOA评分明显高于对照组(P<0.05),而ODI评分明显少于对照组(P<0.05)。结论:经皮椎间孔镜手术患者的VAS评分和ODI评分明显低,而JOA评分明显高。而且患者的手术治疗效果更佳,手术造成的并发症发生率显著低。

Objective:To compare the clinical efficacy of percutaneous transforaminal surgery and laminectomy for lumbar disc herniation.Method:A total of 116 patients with lumbar disc herniation diagnosed and treated in the hospital from January to February 2018 were clinically observed and studied.The patients were randomly divided into groups by digital table extraction.The control group was treated with semi-laminal fenestration.58 patients in the observation group were treated with percutaneous transforaminal surgery.The VAS score,JOA score,ODI score,operation time,hospitalization time,intraoperative blood loss and postoperative complication rate were observed and recorded.Result:After surgery,the VAS score of the observation group was significantly lower than that of the control group,and the hospitalization time and operation time of the observation group were significantly shorter than the control group(P<0.05).The intraoperative blood loss was also significantly lower than that of the control group(P<0.05).Moreover,the incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).At the same time,the JOA score of the observation group was significantly higher than that of the control group(P<0.05),and the ODI score was significantly lower than that of the control group(P<0.05).Conclusion:Patients with percutaneous transforaminal surgery have significantly lower VAS scores and ODI scores,while JOA scores are significantly higher.Moreover,the patients’surgical treatment is better,and the incidence of complications caused by surgery is significantly lower.
