新辅助化疗联合胃癌根治术治疗进展期胃癌的研究

Neoadjuvant Chemotherapy Combined with Radical Gastrectomy for Advanced Gastric Cancer

目的:探讨新辅助化疗联合胃癌根治术治疗进展期胃癌的临床效果。方法:选取2014年6月-2017年6月笔者所在医院收治的98例进展期胃癌患者作为研究对象,其中34例行新辅助化疗联合胃癌根治术,设置为联合组;64例仅行胃癌根治术,设置为对照组。比较两组围手术期指标、疗效、并发症、不良反应、生活质量。结果:两组手术时间、手术出血量、术后肛门排气时间、手术后下床时间和住院时间比较差异无统计学意义(P>0.05)。联合组根治切除率为85.29%(29/34),姑息切除率为14.71%(5/34);对照组根治切除率为62.50%(40/64),姑息切除率为37.50%(24/64),两组比较差异有统计学意义(P<0.05)。两组术后并发症和不良反应的比较差异无统计学意义(P>0.05)。联合组生活质量恢复情况优于对照组,差异有统计学意义(P<0.05)。结论:新辅助化疗联合胃癌根治术可提高进展期胃癌的手术切除率,并可以改善患者术后生活质量。

Objective:To investigate the clinical efficacy of neoadjuvant chemotherapy combined with radical gastrectomy for advanced gastric cancer.Method:From June 2014 to June 2017,98 patients with advanced gastric cancer were treated in our hospital.34 patients were treated with neoadjuvant chemotherapy combined with radical gastrectomy as the combined group,and 64 patients were treated with radical gastrectomy only as the control group.The perioperative indicators,efficacy,complications,adverse reactions and quality of life were compared between the two groups.Result:There were no significant differences in operation time,bleeding volume,anal exhaust time,time to get out of bed and hospital stay between the two groups(P>0.05).The radical resection rate was 85.29%(29/34)and palliative resection rate was 14.71%(5/34)in the combined group,62.50%(40/64)and 37.50%(24/64)in the control group.There were significant differences between the two groups(P<0.05).There were no significant difference in complications and adverse reactions between the two groups(P>0.05).The quality of life recovery in the combined group was significantly higher than that in the control group(P<0.05).Conclusion:Neoadjuvant chemotherapy combined with radical gastrectomy can improve the resection rate of advanced gastric cancer and the quality of life of patients after operation.
