PRP联合闭合复位空心螺钉内固定对股骨颈骨折的治疗作用研究

Therapeutic Effect of PRP Combined with Closed Reduction and Hollow Screw Internal Fixation on Femoral Neck Fracture

目的:探讨PRP(自体生长因子)联合闭合复位空心螺钉内固定对股骨颈骨折的治疗作用。方法:选取2016年1月-2017年12月笔者所在医院收治的100例股骨颈骨折患者,将其随机分为两组,每组50例。单纯手术治疗组选择单纯闭合复位空心螺钉内固定,PRP辅助治疗组选择PRP联合闭合复位空心螺钉内固定。比较两组骨折愈合时间、住院时间,治疗前及治疗后6个月Harris髋关节功能评分、行走功能评分、生存质量评分,并发症发生情况。结果:治疗前两组Harris髋关节功能评分、行走功能评分及生存质量评分比较,差异无统计学意义(P>0.05);治疗6个月后PRP辅助治疗组Harris髋关节功能评分、行走功能评分及生存质量评分均优于单纯手术治疗组,差异有统计学意义(P<0.05)。PRP辅助治疗组骨折愈合时间、住院时间均优于单纯手术治疗组,差异有统计学意义(P<0.05)。PRP辅助治疗组并发症发生率低于单纯手术治疗组,差异有统计学意义(P<0.05)。结论:股骨颈骨折患者实施PRP联合闭合复位空心螺钉内固定效果显著,可有效加快骨折愈合,缩短住院时间,并减少并发症的发生,进一步改善关节功能。

Objective:To investigate the therapeutic effect of PRP combined with closed reduction and hollow screw internal fixation on femoral neck fracture.Method:A total of 100 patients with femoral neck fractures admitted in our hospital from January 2016 to December 2017 were selected and randomly divided into two groups,with 50 patients in each group.The surgical treatment group was treated with closed reduction and hollow screw internal fixation,and the PRP adjuvant treatment group was treated with PRP combined with closed reduction and hollow screw internal fixation.The healing time of fracture,average hospitalization time,Harris hip function score,walking function score and quality of life score and complications before and 6 months after treatment were compared between the two groups.Result:There were no significant differences in Harris hip function score,walking function score and quality of life score between the two groups before treatment(P>0.05).After 6 months of treatment,the Harris hip function score,walking function score and quality of life score of the PRP adjuvant treatment group were better than those of the surgery treatment group,the differences were statistically significant(P<0.05).The healing time of fracture and hospitalization in the PRP adjuvant treatment group were better than those in the surgery treatment group,the differences were statistically significant(P<0.05).The incidence of complications of PRP adjuvant treatment group was lower than that of the surgery treatment group,the difference was statistically significant(P<0.05).Conclusion:PRP combined with closed reduction and hollow screw internal fixation for femoral neck fracture has remarkable effect,which can effectively accelerate fracture healing,shorten hospitalization time,reduce complications and further improve joint function.
