甲泼尼龙联合阿奇霉素治疗小儿难治性肺炎支原体肺炎的临床效果 被引量:11

摘要 目的探讨甲泼尼龙联合阿奇霉素治疗小儿难治性肺炎支原体肺炎的临床效果。方法将本院收治的60例难治性肺炎支原体肺炎患儿采用随机数表法分为常规组和干预组,各30例。常规组给予阿奇霉素治疗,干预组在此基础上给予甲泼尼龙治疗。比较两组的治疗效果。结果干预组治疗总有效率高于常规组(P<0.05)。干预组退热时间、咳嗽缓解时间、肺音消失时间、胸片异常消失时间及住院时间均短于常规组(P<0.05)。治疗2周后,两组C反应蛋白水平及红细胞沉降率均降低,且干预组均低于常规组(P<0.05);两组CD4+水平升高,CD8+水平降低,且干预组优于常规组(P<0.05)。两组不良反应总发生率无显著差异(P>0.05)。结论甲泼尼龙联合阿奇霉素治疗小儿难治性肺炎支原体肺炎效果显著,其可缩短患儿康复进程,减少炎症反应,提高机体免疫功能,且用药安全性良好,值得临床应用。 Objective To investigate the clinical effect of methylprednisolone combined with azithromycin in the treatment of refractory mycoplasma pneumonia in children. Methods Sixty children with refractory mycoplasma pneumonia admitted in our hospital were divided into routine group and intervention group by random number table method, with 30 cases in each group. The routine group was treated with azithromycin, and the intervention group was treated with methylprednisolone on this basis. The clinical effects of the two groups were compared. Results The total effective rate of treatment in the intervention group was higher than that of the routine group(P <0.05). The antipyretic time, cough remission time, pulmonary rale disappearance time, chest X-ray abnormal disappearance time and hospitalization time in the intervention group were shorter than those in the routine group(P<0.05). After 2 weeks of treatment, the levels of Creactive protein and erythrocyte sedimentation rates decreased in both groups, and those in the intervention group were lower than the routine group(P<0.05);the levels of CD4^+increased, and the levels of CD8^+decreased in both groups, and those in the intervention group were better than the routine group(P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups(P >0.05). Conclusion Methylprednisolone combined with azithromycin in the treatment of children with refractory mycoplasma pneumonia has a significant effect, which can shorten the rehabilitation process of children, reduce inflammation reactions and improve the immune function of the body, with good drug safety. It is worthy of clinical application.
