乳腺导管内乳头状瘤124例临床分析

Breast intraductal papilloma,124 cases of clinical analysis

目的探讨乳腺导管内乳头状瘤的临床特征,诊断方法及治疗措施。方法收集2011年1月至2014年12月经手术病理证实为乳腺导管内乳头状瘤的患者124例,进行回顾性分析。结果本组124例患者,以乳头溢液为首发症状为64例,占51.6%,乳腺彩超检出率为61%(75/124),乳腺钼靶检出率为0%(0/116),乳管镜检出率为100%(20/20)。124例均由石蜡病理确诊(单发乳头状瘤116例,多发乳头状瘤8例,伴不典型增生35例,合并乳腺癌2例),其中术中冰冻病理确诊出104例,确诊率为83.9%。本组恶变率为1.6%。结论乳腺导管内乳头状瘤多以乳头溢液为首发症状,乳管镜检查有较高的确诊率,因其有恶变可能,应尽早行手术治疗。

Objective To explore the clinical features of breast intraductal papil oma,diagnostic methods and treatment measures.Methods 124 patients with breast intraductal papil oma which were confirmed by operation and pathology in our hospital from 2011 to 2014 were col ected to analyzed retrospectively.Results There were 64 patients with nipple discharge as the main performance in this group,accounting for 51.6% .The detection rate of color doppler ultrasound examination was 61% (75 /124),the detection rate of mammary gland molybdenum target examination was 0% (0/116),the de-tection rate of ductoscopy examination was 100% (20/20).124 cases of patients with intraductal papil oma was confirmed byparaf in pathology (116 cases of solitary intraductal papil oma,8 cases of multiple intraductal papil oma ,35 cases of intraductal papil oma with atypical ductal hyperplasia,2 cases of intra-ductal papil oma with breast cancer).104 cases in 124 cases was confirmed by frozen pathological,the detection rate was 83.9%.Malignant transformation rate was 1.6%.Conclusion Nipple discharge is the most common clinical symptom in patients with intraductal papil omas.Ductoscopy has a higher coinci-dence rate in diagnosis of the disease .Because of breast intraductal papil oma with a certain rate of malignant Transformation,patients with breast intraduc-tal papil oma should have surgical treatment as early as possible after the diagnosis confirmed.
