哮喘儿童巨噬细胞吞噬功能和肺功能的关系研究 被引量:5

摘要 目的探讨哮喘儿童巨噬细胞吞噬功能和肺功能的关系。方法收集2017年10月—2018年10月经检查确诊为哮喘的儿童25例为试验组,同期收集气道炎性反应但非哮喘的儿童25例作为对照组,通过肺功能仪测定2组儿童1秒用力呼气容积(FEV1)、FEV1占预计值百分率(FEV1%)、FEV1/用力肺活量(FVC)等指标;采集2组儿童的痰液标本,通过痰细胞学方法测定嗜酸性粒细胞、中性粒细胞、巨噬细胞等数量;对2组儿童空腹外周静脉取血3 ml,分离血浆后通过流式细胞仪测定CD3+、CD4+、CD8+等细胞免疫功能,分析IL-12β、iNOS、TNF-αmRNA的表达量并进行比较分析。结果试验组患儿肺功能指标FEV1、FEV1%、FVC均低于对照组,差异有统计学意义(t=7.003,P=0.001;t=6.125,P=0.006;t=5.998,P=0.006);试验组患儿气道痰液中的巨噬细胞和中性粒细胞高于对照组,差异有统计学意义(t=7.105,P=0.001;t=-6.852,P=0.001),痰液嗜酸性粒细胞低于对照组(t=-7.543,P=0.000);试验组血浆T淋巴细胞亚群CD3+、CD4+、CD8+值明显低于对照组,差异均有统计学意义(t=5.694,P=0.001;t=6.853,P=0.005;t=7.165,P=0.003);试验组患儿血浆炎性因子IL-12βmRNA和TNF-αmRNA表达高于对照组(t=8.203,P=0.000;t=7.415,P=0.001),iNOS mRNA表达低于对照组(t=5.496,P=0.001)。结论哮喘儿童巨噬细胞吞噬功能和肺功能之间存在相关性。 Objective To investigate the relationship between macrophage phagocytosis and pulmonary function in asthmatic children.Methods Twenty-five children who were diagnosed with asthma from October 2017 to October 2018 were enrolled in the study group.Twenty-five children who received airway inflammatory response but not asthma was used as a control group.Two groups of children were measured by pulmonary function test for 1 second.Forced expiratory volume(FEV1),FEV1 as predicted percentage(FEV1%),FEV1/forced vital capacity(FVC),etc.;sputum samples from 2 groups of children were collected,and eosinophils were determined by sputum cytology,neutrophils,macrophages,etc.;3 groups of children in the fasting peripheral vein blood 3 ml,separation of plasma,flow cytometry to determine CD3+,CD4+,CD8+and other cellular immune function,analysis of IL-12β,iNOS,TNF-αexpression level of mRNA was analyzed and compared.Results The lung function indexes FEV1,FEV1%and FVC of the experimental group were lower than the control group,and the difference was statistically significant(t=7.003,P=0.001;t=6.125,P=0.006;t=5.998,P=0.006);The macrophages and neutrophils in the airway sputum of the experimental group were higher than those of the control group,and the difference was statistically significant(t=7.105,P=0.001;t=-6.852,P=0.001),eosinophils.The cells were lower than the control group(t=-7.543,P=0.000).The plasma T lymphocyte subsets in the experimental group were significantly lower than the control group(t=5.694,P=0.001;t=6.853,P=0.005;t=7.165,P=0.003);the expression of plasma inflammatory factors IL-12βmRNA and TNF-αmRNA in the experimental group was higher than that in the control group(t=8.203,P=0.000;t=7.415),P=0.001),iNOS mRNA expression was lower than the control group(t=5.496,P=0.001).Conclusion There is a correlation between macrophage phagocytosis and lung function in asthmatic children.
