推拿结合温针灸治疗早期原发性冻结肩对照研究

A Controlled Study of the Effect of Massage Combined with Moxa Acupuncture on Early-stage Primary Frozen Shoulder

目的:观察推拿结合温针灸治疗原发性冻结肩的临床疗效。方法:将原发性冻结肩患者60例随机分为3组:推拿组20例,应用推拿手法治疗;温针灸组20例,应用温针灸治疗;推拿结合温针灸组20例,应用推拿结合温针灸治疗。3组均治疗1次/d,1周为1个疗程,共治疗4个疗程。于治疗前、每疗程结束后分别采用总体疗效评价、VAS视觉疼痛量表、肩关节活动度(PROM)评分进行疼痛和肩关节活动能力评价。结果:治疗前3组的VAS、PROM评分组间差异均无统计学意义(P>0.05);治疗后3组的VAS、PROM评分均分别高于治疗前,组内差异均有统计学意义(P<0.05);推拿组治疗1、2个疗程后的VAS评分均高于温针灸组(P<0.05);推拿结合温针灸组治疗后的VAS评分均高于推拿组、温针灸组(P<0.05);推拿组治疗3、4个疗程后的PROM评分均高于温针灸组(P<0.05);推拿结合温针灸组治疗PROM评分均高于推拿组、温针灸组(P<0.05)。总体疗效评价,推拿组结合温针灸组总有效率为95%,推拿组、温针灸组总有效率均为90%(P<0.05)。结论:温针灸止痛效果优于推拿治疗,推拿更有助于改善肩关节活动度,而推拿结合温针灸综合治疗方案相对于推拿、温针灸单一疗法能显著提高原发性冻结肩患者的总体疗效。

Objective:To observe the clinical effect of Massage combined with Moxa acupuncture in the treatment of Primary frozen shoulder.Methods:Sixty patients with Primary frozen shoulder were randomly divided into three Groups:Massage Group(20cases),treated by Massage therapy;control Group(20cases),treated by Moxa acupuncture;Combination Group(20cases),treated by Massage and Moxa acupuncture.Before and after treatment,the scores of VAS and Shoulder passive motion score were assessed for pain and Shoulder motion.Results:Before treatment:there was no significant difference in vasconstant score among the three Groups(P>0.05).After treatment,the VAS and Shoulder passive motion score of the three Groups were higher than those before treatment,and the differences were statistically significant(P<0.05).The VAS score of Massage Group was higher than that of Moxa acupuncture Group(P<0.05).The VAS score of Massage combined with Moxa acupuncture Group was higher than that of Massage Group(P<0.05).The Shoulder passive motion score of Massage Group was higher than that of Moxa acupuncture Group(P<0.05).The Shoulder passive motion score in the Massage combined with Moxa acupuncture Group was higher than that in the massage Group(P<0.05).Conclusion:the analgesic effect of Moxa acupuncture is better than that of Massage,and Massage is more helpful to improve the motion of shoulder joint,and the combination of Massage and Moxa acupuncture is better than that of Massage.Massage combined with Moxa acupuncture can significantly improve the overall efficacy of primary frozen shoulder patients.r?
