普通冰冻血浆血浆置换治疗早期肝衰竭的疗效观察

Efficacy of plasma exchange using ordinary frozen plasma in treatment of patients with liver failure in early stage

目的分析普通冰冻血浆血浆置换治疗早期肝衰竭患者的疗效。方法将58例符合诊断的早期肝衰竭患者随机分成研究组29例和对照组29例,对照组给予综合护肝治疗,研究组在对照组基础上给予普通冰冻血浆血浆置换治疗。观察2组患者治疗前后丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、胆汁酸(TBA)、总胆红素(TBiL)、间接胆红素(DBiL)、白蛋白(ALB)、凝血酶原时间(PT)、凝血酶原活动度(PTA)、国际标准化比值(INR)的变化,治疗总有效率。结果研究组症状改善率高于对照组(P<0.05);研究组治疗后ALT、AST、TBA、TBiL、DBiL、PT、PTA、INR指标比对照组明显改善(P<0.05);研究组总有效率75.8%,显著优于对照组的51.7%(P<0.05)。结论综合护肝治疗联合普通冰冻血浆血浆置换技术治疗早期肝衰竭疗效明显,可显著改善患者的肝功能和凝血指标,而且安全性好。

Objective To explore the effect of ordinary frozen plasma in plasma exchange for early hepatic failure.Methods Fifty-eight patients with early hepatic failure who met the diagnosis were randomized into the study group(29 cases)and the control group(29 cases).The control group was treated with integrated medical treatment,and the group was combined with normal frozen plasma for plasma replacement therapy. To observe the changes of alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bile acid(TBA),total-bilirubin(TBi L),direct-bilirubin(DBi L),Plasma Albumin(ALB),prothrombin time(PT),prothrombin activity(PTA)and PT-international standardization ratio(INR)before and after treatment in 2 groups,Rate of discharge at discharge,Analysis of clinical effects. Results After treatment,the study group ALT,AST,TBA,TBIL,DBi L,PT,PTA and INR were significantly improved compared with the control group(P<0.05).The efficiency of the study group was significantly higher than that in the control group.Conclusion Comprehensive liver protection therapy combined with common frozen plasma for plasma exchange in the treatment of early liver failure has obvious effect,significantly improve the liver function and blood coagulation function,and is safe.
