摘要 目的了解济南市城市社区女性居民骨质疏松症患病情况,分析相关危险因素及保护因素,为骨质疏松防治提供依据。方法 2017年随机选取选取济南中心城区6个社区女性常住居民,通过问卷调查获得个人基本情况、生活方式相关因素及骨质疏松危险因素等;用超声骨密度仪进行骨密度检测。采用SPSS 20.0软件进行χ~2检验和多因素Logistic回归。结果共调查613名女性,平均年龄为(55.37±11.81)岁。骨量减少检出率为49.10%(301人);骨质疏松症检出率为21.53%(132人),各年龄组检出率分别为5.85%、22.04%、31.64%,差异有统计学意义(P<0.01)。多因素Logistic回归显示高年龄(>60岁)(OR=1.49,95%CI=1.15~2.03)、轻微碰撞(OR=2.23,95%CI=1.26~4.01)、少钙少奶摄入(OR=3.83,95%CI=2.05~6.91)、少维生素D摄入(OR=2.72,95%CI=1.53~4.81)为女性骨质疏松的危险因素(P<0.01),经常运动(OR=0.11,95%CI=0.06~0.23)为保护因素(P<0.01)。结论济南市2017年城市社区女性居民骨质疏松检出率随年龄增加而升高,加强运动、增加钙奶及维生素D的摄入可预防和减缓骨质疏松的发生。 Objective To understand the prevalence of bone loss and osteoporosis among the urban female community residents,and analyze the associated risk and protective factors,so as to provide the basis for the prevention and treatment of osteoporosis.Methods Female permanent residents from 6 communities in the central area of Jinan city were randomly selected in 2017.Questionnaire survey was conducted to obtain the basic information,associated factors of lifestyle and risk factors of osteoporosis;in-place measurement of bone mineral density was detected using an ultrasound absorptiometry.χ~2 test and multivariate logistic regression were used to analyze the data by software SPSS 20.0.Results A total of 613 females were surveyed,with the average age of(55.37±11.81)years old.The detection rate of osteopenia and osteoporosis was 49.10%(301 people)and 21.53%(132 people),respectively.The detection rate was 5.85%,22.04%and 31.64%among the 3 age groups,with statistically significant difference(P<0.05).Multivariate logistic regression analysis showed that age(>60 years)(OR=1.49,95%CI=1.15-2.03),slight collision(OR=2.23,95%CI=1.26-4.01),less calcium and milk intake(OR=3.83,95%CI=2.05-6.91)and less vitamin D intake(OR=2.72,95%CI=1.53-4.81)were the risk factors,while taking regular exercise(OR=0.11,95%CI=0.06-0.23)was the protective factor of female osteoporosis.Conclusion The detection rate of osteoporosis in the urban female community residents in Jinan city increased with age in 2017.Taking regular exercise and increasing milk and vitamin D intake are effective to prevent and slow down the occurrence of osteoporosis.
