Deauville五分法用于18F-FDGPET/CT在外周T淋巴瘤预后评估中的应用

Prognosis value of 18F-FDG PET/CT in patients with peripheral T-cell lymphoma using Deauville criterion

目的:根据Deauville五分法探讨18F-FDG PET/CT在外周T细胞淋巴瘤(PTCL)疗效评估中的应用价值。方法:回顾性分析28例化疗结束后的PTCL患者,根据Deauville标准对18F-FDG PET/CT显像结果进行评分判断,并与病理及临床随访结果进行对照分析。结果:病理及随访结果证实14例完全缓解、12例为疗效未达完全缓解,1例为淋巴结反应性增生,1例为胃癌伴淋巴结转移。18F-FDG PET/CT对PTCL疗效评估的灵敏度、特异度、阳性预测值、阴性预测值及准确率分别为100%、87.5%、85.7%、100%、92.9%,与病理及临床随访结果一致性良好(Kappa=0.857,P<0.05)。结论:Deauville五分法用于18F-FDG PET/CT在PTCL疗效评估中具有重要的临床价值。

Objective To investigate the prognostic value of Deauville 5-point scoring in patients with PTCL. Methods A total of 28 PTCL cases were retrospective analyzed after treatment. Points of 18 F-FDG PET/CT imaging according to Deauville criterion, lymph node biopsies, and clinical summary were compared. Results Eighteen cases were confirmed as CR. Among other 14 positive cases, 1 case of lymph node reactive hyperplasia and 1 of gastric cancer with lymph node metastasis were recorded. Sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of 18 F-FDG PET/CT were 100%、87.5%、85.7%、100%、and 92.9% respectively. 18 F-FDG PET/CT showed concordant results with pathological and clinical follow-up findings(Kappa=0.857,P<0.05). Conclusions 18 F-FDG PET/CT is of important clinical value on the prognosis of PTCL according to the standard of Deauville criterion.
