阿伦膦酸钠和辛伐他汀对绝经女性骨密度及颈动脉内膜厚度的影响

Effect of alendronate and simvastatin on bone mineral density and CIMT in postmenopausal women

目的:分析阿伦膦酸钠和辛伐他汀联合治疗对绝经后女性骨密度及颈动脉内膜厚度(carotid artery intima thickness,CIMT)的影响。方法:研究于我院体检的颈动脉超声显示有动脉粥样板块形成、骨密度测定仪显示骨质疏松的绝经女性,依据病情告知及知情同意原则,按照患者自愿选用的治疗方案,分为阿伦膦酸钠和辛伐他汀联合治疗组、阿伦膦酸钠治疗组、辛伐他汀治疗组,三组观察对象分别为163例、98例、397例,疗程均为一年,比较三组患者治疗前后的右前臂中远1/3处骨密度及颈动脉内膜厚度的变化。结果:联合治疗组与阿伦膦酸钠组治疗后与治疗前骨密度增加差异显著,两组间差异不显著,与辛伐他汀组治疗后差异具有统计学意义(P<0.05),辛伐他汀组治疗后与治疗前骨密度增加差异不显著。联合治疗组、辛伐他汀组颈动脉内膜厚度治疗后与治疗前差异显著(P<0.05),两组间差异不显著,与阿伦膦酸钠组治疗后差异具有统计学意义(P<0.05),阿伦膦酸钠组治疗后与治疗前颈动脉内膜厚度差异不显著。结论:阿仑膦酸钠与降血脂药物辛伐他汀联合治疗绝经后女性骨质疏松具有良好的疗效,增加骨密度的同时可以降低心血管疾病的风险。

Objective To analyze the impact of alendronate and simvastatin combination therapy on bone mineral density in postmenopausal women with osteoporosis and carotid artery intima thickness. Methods 658 cases of postmenopausal osteoporosis women carotid ultrasound showed atherosclerotic plate form, according to the choice of treatment in patients with a voluntary program, divided into alendronate and simvastatin combination therapy group, alendronate treatment group, simvastatin group. A course of a year, the change of distal 1/3 of the right forearm bone mineral density and carotid artery intima-media thickness of three groups patients were analysed. Results Combined treatment group and the alendronate group bone mineral density was increased significantly, and the two groups were not significant differences and compared with simvastatin group was statistically significant. Simvastatin treatment group bone mineral density difference was not significant. The change of combined treatment group and the simvastatin group CIMT was significantly, and between the two groups was not significantly different, and the difference was statistically significant compared with alendronate treatment group. The change of alendronate group before treatment CIMT was not significantly different. Conclusion Alendronate and simvastatin treatment of postmenopausal women with osteoporosis have a good effect, and can reduce the risk of cardiovascular disease.
