^(18)F-FDG PET/CT对乳腺癌区域淋巴结转移的诊断效能探讨 被引量:1

摘要 目的:探讨18F-脱氧葡萄糖(18F-FDG)正电子发射计算机断层显像(PET/CT)对乳腺癌区域淋巴结转:探讨移的诊断效能。移的诊断效能。方法:选取疑原发性乳腺癌患者25例,单发乳腺良性病变患者10例(均为女),均行:选取疑原发性乳腺癌患者^(18)F-FDG PET/CT显像。定性或半定量分析显像结果。疑乳腺癌患者均实施手术治疗。显像。定性或半定量分析显像结果。疑乳腺癌患者均实施手术治疗。结果:疑乳腺癌患者术后经病理检查得知:乳腺癌22例,病灶共23个,良性6个。^(18)F-FDG PET/CT显像定性分析诊断灵敏度73.91%(17/23),特异性94.12%(16/17);若将最大标准摄取值(SUVmax)>2.5作为界限,则^(18)F-FDG PET/CT诊断灵敏度为69.57%(16/23),特异性为58.82%(10/17);若将SUVmax>对侧正常乳腺腺体SUVmax的x-+2s作为界值,则PET/CT诊断灵敏度95.65%(22/23),特异性58.82%(10/17);其灵敏度相比前2种方法,明显高于后者(χ~2=4.14,4.13,均P<0.05)。定性方法特异性较半定量分析,显著高于后者(均χ~2=5.6,均P<0.05)。行区域淋巴结廓清术的21例患者经病理检查,有淋巴结转移9例。^(18)F-FDG PET/CT定性诊断灵敏度55.56%(5/9),特异性83.33%(10/12)。结论:乳腺癌^(18)F-FDG PET/CT诊断中,将病灶SUVmax>正常乳腺腺体SUVmax的x-+2s作为界值,并以此开展半定量分析,灵敏度好;定性分析诊断特异性较好。
