摘要
目的:探讨超声引导下神经刺激仪定位臂丛神经阻滞的效果。方法:将2015年5月29日—2017年10月5日期间我院收治的于腋入路臂丛神经阻滞下进行手、上臂、上肢远端的40例手术患者分为两组,对照组20例,仅应用神经刺激仪操作;观察组20例,在超声引导下使用神经刺激仪操作,比较两组患者的臂丛神经阻滞效果与并发症情况。结果:观察组阻滞效果达优者14例、良6例,阻滞有效率100%;对照组阻滞效果达优者7例、良10例、差3例,阻滞有效率85%(P<0.05)。观察组仅有1例患者有出血与血肿症状,并发症发生率5%;对照组出现局麻药毒性反应、神经损伤各1例、出血与血肿2例,并发症发生率20%(P<0.05)。结论:采用超声引导下神经刺激仪定位臂丛神经,临床阻滞效果理想,安全性高,可作为首选方案予以推广。
Objective To explore the effect of ultrasound-guided neurostimulator to locate the nerve block of brachial plexus. Methods On May 29, 2015 to October 5, 2017 in our hospital during the period of axillary brachial plexus block in the road, upper arm, hand under 40 cases on the far side of upper extremity surgery patients were divided into two groups, control group 20 cases, the only application of nerve stimulator operation; In 20 cases, the neurostimulator was used to compare the effects of brachial plexus nerve block and complications in the two groups.Results There were 14 cases of excellent retardation of the observation group, 6 cases of good resistance and 100%efficiency. In the control group, there were 7 cases, 10 cases and 3 cases, and the block efficiency was 85%(P < 0.05).Only one patient in the observation group had symptoms of hemorrhage and hematoma, and the complication rate was 5%. In the control group, there were 2 cases of drug toxicity, 1 case of nerve injury, 2 cases of hemorrhage and hematoma, and 20% of the complication rate(P < 0.05). Conclusion Using the ultrasonic guided neurostimulator to locate the brachial plexus nerve, the clinical retardation effect is ideal, the safety is high, can be promoted as the preferred plan.
