经颅多普勒联合颈动脉超声与数字减影血管造影在缺血性脑血管疾病诊断中的对比分析 被引量:19

摘要 目的:探究经颅多普勒(TCD)和颈动脉超声联合同数字减影血管造影(DSA)诊断缺血性脑血管疾病的效果。方法:选取2016年8月—2017年8月收治的110例疑似缺血性脑血管疾病患者资料进行分析,所有研究对象均行DSA、TCD及颈动脉超声检查,将行DSA诊断者作为对照组,行TCD与颈动脉超声诊断者作为研究组,比较两组血管狭窄检出情况、TCD联合颈动脉超声诊断效能。结果:对照组共检出血管狭窄90例,其中闭塞13例、重度16例,中度20例,轻度41例,与研究组无显著差异性(P>0.05);将对照组DSA诊断作为"金标准",研究组诊断特异度、灵敏度、准确度分别为95.00%、91.11%、91.81%。结论:动脉超声与TCD联合诊断具有较高诊断效能,与DSA诊断准确率无显著差异,可指导缺血性脑血管疾病临床治疗方案的制定。 Objective To explore the effect of combined use of cranial Doppler(TCD) and carotid ultrasonography in the diagnosis of ischemic cerebrovascular diseases with digital subtraction angiography(DSA). Methods The data of 110 suspected ischemic cerebrovascular diseases admitted from August 2016 to August 2017 were analyzed. All the subjects underwent DSA, TCD and carotid ultrasonography, and the DSA diagnosis was used as the control group, Line TCD and carotid ultrasound diagnosis as a research group, compared the detection of vascular stenosis, TCD combined carotid ultrasound diagnostic efficacy. Results 90 cases of vascular stenosis were detected in the control group, of which 13 cases were occluded,16 cases were severe, 20 cases were moderate and 41 cases were mild. There was no significant difference between the two groups(P> 0.05) "Gold standard", the study group diagnosis specificity, sensitivity and accuracy were 95.00%, 91.11%,91.81%. Conclusion The combined diagnosis of arterial ultrasound and TCD has high diagnostic efficacy, and DSA diagnostic accuracy was no significant difference, can guide the development of clinical treatment of ischemic cerebrovascular disease program.
