摘要
目的:探讨研究彩色多普勒超声评估糖尿病下肢动脉粥样硬化病变的价值。方法:对2014年10月—2017年10月期间,以我院收治的120例糖尿病患者作为研究组,进行彩色多普勒超声双下肢动脉检查,另设同一时期120例非糖尿病患者的双下肢动脉彩色多普勒超声检查作为对照组。观察对比两组患者双侧下肢动脉粥样硬化斑块形成率及患者双侧下肢的血管的管壁、管腔、内-中膜的厚度和斑块的大小及其血流情况等。结果:研究组下肢动脉血管的内中膜增厚、硬化斑块、管腔狭窄及闭塞的发病率要高于对照组患者,P<0.05,差异具有统计学意义;观察所见,研究组患者对比对照组患者的双下肢的动脉均有不同程度的狭窄,其中以足背动脉、胫前动脉的狭窄差异具有统计学意义(P<0.05)。研究组患者双下肢血管的内-中膜厚度均厚于对照组患者,P<0.05,差异具有统计学意义。结论:糖尿病患者易多发动脉粥样硬化,早期常常累及肢体远端血管,应用彩色多普勒超声可以准确观察双下肢动脉硬化的部位、范围以及程度,有利于诊断糖尿病下肢动脉病变。
Objective To explore the study of color Doppler ultrasound assessment of diabetic lower extremity atherosclerotic lesion value. Methods On the October 2014-2017 year in October, with 120 cases of diabetic patients in our hospital as the study group, the lower limbs artery by color Doppler ultrasound examination, the lower extremity artery color Doppler ultrasound examination during the same period in 120 cases diabetic patients as the control group. The tube wall, and the rate of patients with bilateral lower limb vascular formation were observed in the two groups of patients with bilateral lower extremity atherosclerotic plaque lumen, intima-media thickness and plaque size and blood flow. Results The incidence of intima-media thickening, atherosclerotic plaque, lumen stenosis and occlusion in the study group was significantly higher than that in the control group(P < 0.05). It was observed that the arteries of the lower limbs of the patients in the study group had different degrees of stenosis compared with those of the patients in the control group, in which the dorsal foot artery was the main artery. The difference of stenosis of anterior tibial artery was statistically significant(P<0.05). The intima-media thickness of both lower extremity vessels in the study group was thicker than that in the control group(P<0.05). Conclusion patients with diabetes are prone to multiple atherosclerosis and often involve distal blood vessels in the early stage.Color Doppler ultrasound can be used to accurately observe the position, range and extent of arteriosclerosis in the lower extremities, which is beneficial to the diagnosis of diabetic lower extremity artery disease.
