摘要
目的:探讨声触诊组织成像量化技术(VTQ)检测动脉斑块稳定性的临床价值。方法:常规超声检测80例确诊颈动脉斑块的患者,确定97个目标斑块,之后进行二维超声造影(2D-CEUS)及VTQ检查,对斑块的超声造影特点及剪切波速度(SWV)值进行分析。结果:97个目标斑块根据声学特征分为两组,均质斑块组42个和不均质斑块组55个,均质斑块按回声特点又分为低回声斑块组18个,等回声斑块组11个,强回声斑块组13个。97个目标斑块根据超声造影特点分为易损斑块组36个,稳定斑块组44个,易损斑块组SWV低于稳定斑块(P<0.05);ROC曲线下面积0.856,以2.54m/s为界值,VTQ技术诊断斑块稳定性的灵敏度为72.4%,特异度为84.6%。结论:VTQ技术对动脉斑块的稳定性有较好的诊断价值。
Objective To explore the clinical value of Detection the stability of the artery plaques with virtual touch tissue quantification imaging technique. Methods 80 patients with carotid artery plaque were diagnosed by conventional ultrasonography, Identify 97 target plaques, then two dimensional ultrasound(2 D-CEUS) and VTQ were performed. The ultrasonography characteristics and shear wave velocity(SWV) of the plaques were analyzed. Results The 97 target plaques were divided into two groups based on acoustic characteristics, there were 42 homogeneous plaques and 55 heterogeneous plaques, The homogeneous plaques were divided into 18 low-echo patches, 11 Isoechoic plaque group and 13 Strong echo plaque group by echo characteristics. The 97 target plaques were divided into 36 vulnerable patch groups and 44 Stable plaques according to the characteristics of ultrasonography. The SWV of the vulnerable patch group was significantly lower than the stable plaque, difference was statistically(P<0.05). Under ROC curve was 0.856, with 2.54 m/s as the boundary value. The sensitivity of VTQ technology to the diagnosis of plaque stability was 72.4%, and the specificity was 84.6%. Conclusion VTQ technology has good diagnostic value for arterial plaque stability.
