经颅多普勒超声在颈动脉重度狭窄或闭塞后脑侧支循环代偿中的应用 被引量:4

摘要 目的:评价经颅多普勒超声(TCD)诊断颈动脉重度狭窄或闭塞后脑侧支代偿能力的敏感度和特异度。方法:经数字减影血管造影(DSA)证实为单侧颈动脉重度狭窄或闭塞患者64例,以DSA对颈动脉侧支的诊断为金标准,计算TCD测得的病侧颈动脉与健侧颈动脉峰值血流速度比值(d VACA/n VMCA)及病侧颈动脉与健侧颈动脉交通后段的峰值血流速度比值(d VPCA/n VPCA),比较有无颈动脉代偿时,各指标的不同。通过受试者工作特征(ROC)曲线分析,确定TCD参数评估标准。结果:通过TCD检测d VACA/n VMCA。来判断有无前软颈动脉侧支代偿,绘制ROC曲线,曲线下面积为0.885±0,050,95%CI:0.787~0.983,P<0.01,诊断界点为d VACA/n VMCA>1.00时,敏感度为80%,特异度为87%。通过TCD检测d VPCA/n VPCA来判断有无后颈动脉侧支代偿情况,绘制ROC曲线,曲线下面积为0.821±0.058,95%CI:0.708~0.934,P<0.01。分别以d VPCA/n VPCA>1.20、>1.30、>1.40、>1.50为诊断界点,敏感度和特异度不同,d VPCA/n VPCA越高,诊断的特异度越高,但敏感度越低,诊断界点为d VPCA/n VPCA>1.20时,敏感度为67%,特异度为79%。结论:TCD不仅能够检测初级侧支和颈动脉侧支代偿,也能通过血液动力学信息评价软颈动脉侧支代偿情况。
