侧卧位经皮椎间孔镜下治疗腰椎间盘突出症疗效分析 被引量:2

Effect of the Tateral Decubitus Position on Percutaneous Endoscopic Lumbar Discectomy in the Treatment of Lumbar Intervertebral Disc Herniation

摘要 目的:侧卧位下经皮椎间孔镜下椎间盘切除术(percutaneous endoscopiclumbar diseectomy,PELD)治疗腰椎间盘突出症的疗效。方法:对23例(男16例,女7例)腰椎间盘突出症患者,经3~6个月保守治疗无效后,采用侧卧位下经皮椎间孔镜下行椎间盘切除术,对术后疗效进行回顾性分析。结果:23例手术时间l.5~4.5h,平均2.5h;术后住院时间7~10d,平均8d。23例行3~24个月随访,根据改良Macnab标准:优16例,良5例,可1例,差1例,1例出现椎间盘感染经去甲万古霉素抗炎治疗2周后病愈出院。结论:侧卧位下PELD治疗腰椎间盘突出症疗效满意,具有创伤小、恢复快、并发症少、操作简单等优点。 Objective To explore the effect of the lateral decubitus position, under local anesthesia,on the clinical curative outcomes of percutaneousendoscopic lumbar discectomy(PELD) in the treatment of lumbar intervertebral disc herniation.Methods 23 cases(16 males and 6 females) of patients with lumbar disc herniation Were analyzed restrospectively.All the patients were treated with PELD.Results The mean operative time was 2.5 h(from 1.5 to 4.5 h),and the mean length of stay was 8 d(from 7 to 10 d).All patients were followed Up from 3 to 24 months with an average of 10 months.According to the modified Macnab criteria,16 cases were excellent result,5 good,1 fair,l poor.1 case occured infection after PELD, and recovering after 2 weeks of anti-inflammatory treatment.Conclusion The lateral decubitus position was the ideal position for treatment of PELD.It shows a desirable effect in the treatment of lumbar disc herniation with advantages such as better effect of less trauma,shorter healing time,and fewer postoperative complications,with provides an affirmed efficacy in the treatment of lumbar disc herniation.
