^(18)F-FDG PET/CT对食道癌术后复发及转移中的应用价值

摘要 目的:^(18)F-FDG PET/CT对食道癌术后复发及转移中的应用价值。方法:分析^(18)F-FDG PET/CT对食道癌术后吻合口复发或转移的诊断结果与术后病理或随访结果进行Kappa检验一致性分析;比较分析食道癌术后吻合口复发与炎性病变的SUV早期值、SUV延迟值的差异性。结果:112例食道癌术后患者,经PET/CT检查诊断为复发或转移者97例,无复发者15例,与术后病理及随访结果比较进行Kappa检验,检验结果一致性良好。112例食道癌术后患者中SUV早期值范围在肝组织和脑实质之间者有53例。吻合口复发病灶的SUV早期值为5.4±1.0,SUV延迟值为7.1±1.5,SUV延迟值相较于SUV早期值增高。食道炎性病变或肉芽组织增生病灶的SUV早期值为5.51±1.25,SUV延迟值为4.5±1.6,SUV延迟值相较于SUV早期值减低,差异具有统计学意义。结论:^(18)F-FDG PET/CT可以为食道癌术后吻合口复发及远处转移的监测提供可靠依据,具有重要的使用价值。 Objectivee To evaluate the value of ^(18)F-FDG PET/CT in the recurrence and metastasis of esophageal cancer. Methodss ^(18)F-FDG PET/CT was used to analyze the recurrence or metastasis of anastomotic stoma after surgery for esophageal cancer. Kappa test was used to analyze the consistency of postoperative pathological or follow-up results. Anastomotic recurrence and inflammatory lesions were compared and analyzed after surgery. Differences in SUV early value and SUV retardation value. n value. Results Results Among 112 patients with esophageal cancer, 97 were diagnosed as having recurrence or metastasis by PET/CT examination, and 15 had no recurrence. Kappa test was performed comparing with postoperative pathology and follow-up results. The test results were in good agreement. In 112 cases of esophageal cancer patients, the early value of SUV ranged from liver tissue to brain parenchyma in 53 cases. The early SUV value of recurrent recurrence was 5.4±1.0, and the SUV delay was 7.1±1.5. The SUV delay value was higher than the early SUV value. The early value of SUV in esophageal inflammatory lesions or granulation tissue hyperplasia lesions was 5.51±1.25, and the SUV delay was 4.5±1.6. The SUV delay value was lower than that of the SUV in the early stage, and the difference was statistically significant. e was lower than that of the SUV in the early stage, and the difference was statistically significant. Conclusion Conclusion ^(18)F-FDG PET/CT can provide a reliable basis for the monitoring of anastomotic recurrence and distant metastasis after esophageal cancer surgery, and has important application value.
