CT与MRI对确定跳跃型椎体压缩骨折靶椎体的价值

摘要 目的:探讨MRI与CT在确定跳跃型椎体压缩骨折疼痛靶椎体的一致性。方法:选取2016年3月—2017年2月期间23例跳跃型椎体压缩骨折患者术前CT和MRI影像资料,对比两种检查方法的骨折检出率;结合查体共同确定疼痛靶椎体,行PKP治疗。采用术前及术后疼痛视觉模拟VAS评分,ODI残障指数的改善程度评估手术效果。结果:23例患者MRI检出79个椎体骨折,CT检出68个,差异有统计学意义,P<0.05。73个椎体行PKP治疗;与术前比较,术后VAS、Oswestry残障指数评分均明显改善,差异有统计学意义(P<0.05)。结论:MRI用于跳跃型椎体压缩骨折的诊断,检出率高于CT,两者结合应用,对指导制定手术方案及穿刺路径有实用价值。 Objective To investigate the consistency of MRI and CT in determining the pain target vertebral body of the jumping vertebral compression fracture.Methods The preoperative CT and MRI images were selected for 23 patients with jump type vertebral compression fractures from March 2016 to February 2017.The fracture detection rate was compared. to determined the pain target and treated by PKP,used The VAS score and the Oswestry disability index to evaluate the operation effect.Results In 23 patients,79 vertebral fractures were detected by MRI,68 were detected by CT,PKP was treated in 73 vertebral bodies.Compared with preoperative comparison,the scores of VAS and ODI were significantly improved,the difference was statistically significant(P <0.05).Conclusion MRI used to diagnose the jumping osteoporotic vertebral fractures detection rate is higher than used of CT, The two methods are combination used, It is of practical value to guide the development of surgical procedure and puncture path.
